Title: Nurse Practitioner / Physician Assistant - Home-based Hospital Care Location: Madison, WI, United States (Hybrid) Job Description: Work Schedule: 100% FTE, working 7:00 am – 5:00 pm, Seven days on and seven days off. This will include every other weekend and will include holidays as scheduled. This will be a hybrid position with most clinical hours to be completed virtually after orientation. We are seeking a Nurse Practitioner or Physician Assistant to: - Independently care for the hospitalized patients in the UW Home-based Hospital Care Program. - Work with patients with acute and chronic disease. - Perform assessment, diagnosis, treatment, management, prevention, care coordination including multidisciplinary care rounds, and management of acute and chronic conditions. These may include the following: Adult/Gerontology Acute Care or Adult/Gerontology Primary Care with clinical experience in the hospital or previous ICU experience as Nurse or Nurse Practitioner.
