SGV Home Case Coordinator

Role Description The SGVCOG is seeking to fill one full-time SGV Home Case Coordinator position in the Regional Planning department supporting homelessness prevention efforts through SGV Home. The selected candidate will provide direct client services to households experiencing housing instability and assist participants in accessing financial assistance, community resources, legal services, and other supports designed to promote long-term housing stability. Examples of Duties - Housing Stability Coordination and Case Management: - Provide direct services to households participating in SGV Home and other homelessness prevention programs. - Conduct client assessments and develop individualized housing stability plans. - Maintain an assigned caseload and monitor participation progress. - Connect clients with community resources, benefits, legal services, and support programs. - Housing Assistance Administration: - Evaluate requests for rental assistance and other housing stability services. - Review client circumstances and supporting documentation. - Determine appropriate assistance recommendations and verify ongoing program eligibility. - Prepare required forms and program documentation. - Resource Navigation and Service Coordination: - Work closely with landlords, legal service providers, housing providers, public agencies, social service organizations, and community partners. - Assist participants in resolving barriers to housing stability. - Provide information regarding tenant protections, available resources, and housing assistance programs. - Documentation and Compliance: - Maintain complete and accurate files, records, and documentation. - Enter and track information in program databases. - Prepare reports and correspondence. - Ensure compliance with program guidelines, funding requirements, and agency policies. - Program Support and Outreach: - Support outreach activities, workshops, special projects, and program evaluation efforts. - Collect and analyze program data. - Participate in performance management activities and provide recommendations to improve service delivery and program outcomes. Qualifications - Bachelor’s degree in public administration, business administration, social work, human services, sociology, psychology, community development, urban planning, or a related field. - Two years of experience providing direct client services in housing assistance, homelessness prevention, social services, community services, or a related field. Desired Qualifications - Experience providing direct client services in housing assistance, homelessness prevention, social services, or a related field. - Experience conducting client assessments, developing service plans, and coordinating resources. - Knowledge of housing stability programs, community resources, public assistance programs, and applicable housing regulations. - Strong communication, documentation, organization, and problem-solving skills. - Ability to build effective relationships with clients, landlords, service providers, and community partners. Driving Requirements Driving is essential for this role. This position travels for client meetings, conferences, or community engagements. Recruitment Information The SGVCOG is seeking to fill one position in this recruitment. This position will close on July 17, 2026. Salary The current range for this classification is $73,745 – $101,658. The starting salary is based on qualifications, experience, and internal equity and typically does not exceed the midpoint of the salary range.

each tenant, and review caps, base years, and non-standard exclusions. • Develop initial monthly rental payment schedules. • Ability to assist with monitoring, reviewing and approving all Rent, CAM, Operating Expense, Tax and Insurance payments to Landlords, including reviewing leases to insure accuracy of all charges. • Ability to review and validate Consumer Price Index adjustments as provided in legal documents. • Maintain file systems, including the data integrity of initial information uploaded to the database application used for tracking all Tenant and Lease related data specific to the initial set up of a new account during transition. • Maintain client transition financial forecasts for internal tracking and budgeting. • Confirm and approve monthly client billing. • Responsible for ensuring that all pass-through expenses are billed in accordance with contract and internal process for transitions. • Track A/R for assigned clients and escalate to appropriate parties. • Regular status reports to internal management. • Knowledge of Lease Administration scope of services, master service agreement and intercompany contractual review and development in partnership with Colliers legal, risk and security teams. • Develop and maintain tracking method for team workloads and assignments and provide refreshed gearing annually. • Review and validate team allocations and alignment to contractual scope of work and client’s expectations. • Manage team workloads ensuring equitable distribution. • Develop roadmap to streamline and enhance transition program delivery. • Lead key program initiatives such as Best Practice tracking, Case Study Development, AI partnership with Technology, Global COE Partnerships, etc. • Conduct post transition Lessons Learned to help inform and implement process improvements for future transitions. • Other duties as assigned.
