Role Description As a Care Navigator, you will be an invaluable member of our team, ready and eager to make an impact internally and to patients we serve. You will support patient success by: - Identifying the needs and challenges patients have and assisting in implementing interventions to guide new thinking and create behavior change. - Providing remote monitoring services to help support patients as they progress through their home-exercise therapy program. - Directing and assisting users in therapeutic exercise, neuromuscular re-education, and functional training through our technology platform. - Providing ongoing feedback to the evaluating clinician on patient progress outside of the clinic. - Maintaining necessary records and notes while providing remote monitoring. - Assisting with improving processes and functionality of the platform. - Organizing and managing a caseload while staying efficient. Qualifications - 2+ years of experience in musculoskeletal health is a plus. - Graduated from an accredited physical therapy program. - Active PT license in a compact state (https://www.ptcompact.org/). - Tech Savvy - thrive in a multi-platform environment, comfortably switching between systems (EHR, messaging, scheduling) without missing a beat. - Experience in behavioral modification; Health Coach Certification is a plus. - Excellent communication skills, interpersonal, and presentation skills, and an ability to collect and present meaningful data to drive decisions. - Strong relationship-building and listening skills. - Demonstrated patient focus; a genuine passion for the user and their experience. - Ability to multi-task and manage various types of users. - Ability to work independently with limited supervision. - The flexibility and agility to work cross-functionally in a fast-paced environment. - The ability to influence change and bring enthusiasm and excitement to the workplace. - Independent and proactive work style; quick, flexible thinking; and an open, inquisitive approach. - Eager to make an impact at a young company addressing an important problem. - Passion for health and wellness. Galileo is looking for a culturally-competent Nurse Practitioner to join our Population Health team. In this role, you will use phone and video-based care to provide high-quality, comprehensive follow-up care to patients who are post-hospital discharge. - Cover our care needs for our Transitions of Care program. - Day to day work includes telephonic outreach (“cold calls”) and scheduled video/phone visits for patients who have recently been hospitalized and discharged. - Visits focus on: - Medication reconciliation - Symptom screening - Ordering of any needed follow-up labs, medications or supplies - Navigate conversations with patients to build trust, explain Galileo's care model and connect them to services. - Coordinate care across settings — hospital and ED transitions, pharmacies, and community providers — to keep patients connected and moving forward. - Collaborate with our nursing team. - Be accountable to individual performance targets while contributing to team-level population health goals. Qualifications - Certified Nurse Practitioner in Adult, Family or Geriatrics. - Active state licensure in SC, with the ability to obtain additional state licensures. - Bonus if you have active licenses in TX, NJ, PA, IL, NY, MA, MN, and/or LA. - 5+ years of post-graduate clinical experience in Primary Care or Geriatrics. - 2 of those years may be substituted by time spent working at a Skilled-Nursing facility or Long Term Care facility. - Direct experience engaging Medicare and Medicaid populations managing complex, comorbid conditions. - Motivational interviewing skills you can draw on to help patients make meaningful behavioral changes. - Comfort in a digital-first environment, with experience in or a genuine appetite for virtual care delivery. - Solid tech proficiency; familiarity with Google Suite and Slack is a plus. - Excellent written and verbal communication skills. - Bilingual in English and Spanish, including cultural fluency, is a strong bonus. Requirements - Availability to work 32-36 hours/week. - Work in 4-hour shifts (mornings, afternoons, evenings, overnight on weekdays and weekends). - Providers are expected to work weekends on a monthly basis. Role Description Join our team of talented, mission-driven individuals who are eager to help the children and schools that need it the most. With locations in San Diego, Orange, Los Angeles, Riverside, and Contra Costa Counties, Verbal Behavior Associates (VBA) partners with 20+ school districts to provide ABA therapy. We're continually expanding as the need for language services grows. Your role at VBA: As a Medical Scheduling Coordinator, you will create and coordinate weekly schedules for our therapists and clients, matching them together based on location and therapy needs. If you have high-volume scheduling experience and enjoy ‘putting the pieces of the puzzle together’, you’ll thrive in this role.
