Licensed Physical Therapy Assistant, Care Navigator

Net Health

• Provide remote monitoring services to help support patients as they progress through their home-exercise therapy program • Direct and assist users in therapeutic exercise, neuromuscular re-education, and functional training through our technology platform • Provide ongoing feedback to the evaluating clinician on patient progress outside of the clinic • Maintain necessary records and notes while providing remote monitoring • Assist with improving processes and functionality of the platform • Organize and manage a caseload while staying efficient

Job Requirements

  • 2+ years of experience in musculoskeletal health is a plus
  • Graduated from an accredited physical therapy program
  • Active PT license in a compact state (https://www.ptcompact.org/)
  • Active PTA license in Michigan
  • Tech Savvy - thrive in a multi-platform environment, comfortably switching between systems (EHR, messaging, scheduling)
  • Experience in behavioral modification, Health Coach Certification is a plus
  • Excellent communication skills, interpersonal, and presentation skills, and an ability to collect and present meaningful data to drive decisions
  • Strong relationship-building and listening skills
  • Demonstrated patient focus; a genuine passion for the user and their experience
  • Ability to multi-task and manage various types of users
  • Ability to work independently with limited supervision
  • The flexibility and agility to work cross-functionally in a fast-paced environment
  • The ability to influence change and bring enthusiasm and excitement to the workplace
