Role Description Du arbeitest in unserer Caspar Clinic in einem interdisziplinären Team aus Therapeut*innen unterschiedlicher Fachrichtungen, ärztlichen und psychotherapeutischen Kolleg*innen. - Mithilfe der Caspar-App, Chat und Telefon werden Patient*innen in verschiedenen Nachsorgekonzepten (z.B. IRENA, IRENA Neuro, Prävention, Adipositas) von Dir individuell betreut. - Nach einem persönlichen telefonischen Begrüßungsgespräch erstellst Du je nach Diagnose, Bedürfnissen und Wünschen des Patienten einen individuellen Trainingsplan in unserer Anwendung. - Du motivierst Deine Patient*innen, um die Therapietreue zu erhöhen. - Gerne darfst Du bei uns im Team der Patient*innnenbetreuung kreativ eigene Ideen einbringen und helfen, neue Strukturen und Prozesse zu etablieren. Qualifications - Du bringst eine abgeschlossene Berufsausbildung/ Studium zum / zur Therapeut*in (Physiotherapeut*in, Sportwissenschaftler*in) mit. - Du hast bereits mindestens 2 Jahre Berufserfahrung als Therapeut*in mit Patient*innen aus dem Reha-Bereich. - Du konntest neben der Orthopädie auch einschlägige Erfahrungen in der Onkologie sammeln. - Du hast eine Affinität zu digitalen Medien und ihrer Nutzung im Gesundheitswesen. - Du bist empathisch, kommunikationsstark und wertschätzend. - Du sprichst fließend Deutsch und hast zumindest Grundkenntnisse in Englisch.
