For this position, Physician Assistants will manage patients in an outpatient clinic setting as well as providing in-home clinical support for patients in the community. GGH PA's will be supporting clients with their patient panels and partnering with clinics as an embedded member of the care team with the goal of preventing unnecessary acute care admission, providing holistic patient care, optimizing the management of the patients' medical conditions, and providing support for clinical and psychosocial factors that contribute to the cyclical utilization of healthcare. What will you do as Nurse Practitioner at GGH? - Assess patients' clinical problems and healthcare needs and provide clinical care and consultation - Perform an annual wellness exam for senior Medicare patients in a designated clinic or at their place of residence. Obtain and record the patient's medical history and symptoms, including medicine reconciliation - HCC coding, verification, and risk adjustment - Coordinates continuity of care, prevention and avoidance of complications, and close clinical treatment and management under the direction of the patient's primary healthcare provider(s) and collaboration with other referral sources - Perform various tests and screenings - Counseling patients with education around their discharge plan - Execute the discharge plan and use your critical thinking skills to build an effective care plan during a critical transition period after a hospital discharge - Supports patients and their families, and provides compassionate care - Contribute to an interdisciplinary team (IDT) model and attend regular IDT meetings to collaborate with other healthcare professionals (social workers, case managers, and other clinicians) to ensure the holistic approach to the care plan is carried out - Collaborate with care team to ensure completion of holistic patient care - Ability to travel within the Jackson County, OR area 5 days a week to provide care to patients in their home based on the needs of business/market - Expected patient volume completed per day in clinic: 8-10 - Expected patient volume completed per day in-home: 5-6 - Complete charts within 72 hours of patients' visits; you will have access to EMR and/or portals to review charts and document notes - Cover for other APPs and Care Team Members when necessary - Maintains patients' confidence by completing and safeguarding medical records, keeping patients' information confidential, and abiding by all HIPAA rules and regulations This position is a part time Physician Assistant opportunity. Role Description The Senior Risk Investigator is a key individual contributor within Paysafe’s Merchant Risk function, responsible for the proactive monitoring, investigation, and management of fraud and payment risk across the merchant portfolio. This role requires deep expertise in fraud, financial crime, and card payments, combined with strong analytical judgment and decision-making capability. You will manage your own caseload, take ownership of complex and high-risk investigations, and make informed, risk-based decisions to protect the business from financial and regulatory exposure. As a senior member of the team, you will also play a critical role in supporting escalations, providing guidance to more junior investigators, and contributing to the continuous improvement of risk processes and controls. Responsibilities - Monitor daily merchant activity to identify unusual patterns, fraud indicators, and emerging risks - Investigate complex or high-risk transaction activity across merchants, working with issuing banks, cardholders, and other partners where required - Apply sound judgment to determine appropriate actions, including account restrictions, funding holds, or escalation - Manage your own case portfolio, ensuring timely, accurate, and well-documented decision-making - Act as a senior point of support for complex or escalated cases within the team - Demonstrate strong decision-making capability, with clear rationale aligned to policy and risk appetite - Communicate effectively with a range of stakeholders, including merchants, issuing banks, sponsor banks, sales teams, and internal functions - Provide day-to-day guidance and informal coaching to junior investigators - Assist with workflow coordination and case allocation where needed Qualifications - Bachelor’s Degree in financial, business or economics; or an equivalent combination of education and experience - Five plus (5+) years’ experience in risk, fraud and financial crime in Payments - Demonstrate ability to perform day-to-day fraud investigation and risk activities for merchant accountants - A self-starter that requires minimal supervision - Strong organizational and analytical skills - Experience mentoring and coaching more junior Investigators - Ability and willingness to take escalated cases and/or merchant calls, when required - Excellent communication (written and oral) and interpersonal skills - Strong attention to detail and ability to make real-time, informed decisions - Ability to work in a team-driven environment yet work independently to complete daily expectations and deadlines Benefits - One network.
