Benefits: - Sign-On Bonus $2,000 - 401(k) with Employer Match - Health, Dental, & Vision Insurance - Disability and Life Insurance - Flexible Spending Account, Health Savings Account, and Dependent Care Account Options - Paid Time Off - 13 Paid Holidays - Annual increase with Profit Sharing - Hybrid Office/Work from Home Model - Mileage reimbursement - Flexibility Care Manager (Full-time Exempt, Monday-Friday 40 hours/week) Key Responsibilities: - Assessment and Evaluation - Conduct initial and ongoing assessments of AFC qualified settings, including interviews, home studies, home inspections, and medical clearance of care providers. - Obtain social history and conduct psychosocial assessments and evaluations. - Caregiver Management - Select, train, evaluate, and supervise AFC caregivers in collaboration with registered nurses. - AFC Plan of Care - Participate in the development, implementation, and ongoing review of the AFC plan of care. - On-Site Visits - Conduct on-site visits with each member at the qualified setting bimonthly for Level I and monthly for Level II, alternating with the registered nurse. - Documentation and Progress Notes - Complete a care management progress note corresponding with each on-site visit or encounter and upon significant change. - Periodically review AFC caregiver logs. - Health and Community Services - Assist with obtaining information and accessing other health-care and community services for individuals under care. - Safety Procedures and Discharge Planning - Review and document fire and safety procedures for qualified settings. - Participate in discharge planning and implementation. - Annual Review - Review at least annually the suitability of the qualified setting. - Additional Duties - Complete intake forms in conjunction with the registered nurse. - Prior to placement, complete all required Nonotuck paperwork for both care provider and person served in conjunction with the RN. Qualifications: - Bachelor's Degree required. - Social Work License preferred. - Bilingual Spanish and/or Portuguese a plus. - Two years experience with elders, those who have been diagnosed with ID/D, ABI, or who have chronic conditions affecting their ability to live safely in the community. - Strong organizational and interpersonal skills. - Knowledge of AFC and DDS regulations and guidelines. - Ability to work collaboratively with a multidisciplinary team. - Attention to detail and organizational skills. - Valid driver's license with an excellent RMV record (subject to RMV background check). - Background check required. The Clinical Team leads the deployment of CoMind's technology into hospital environments for the first time, managing clinical sites, supporting device installations, and ensuring the smooth execution of clinical studies in partnership with clinicians. As Senior CRA, you will own site monitoring across CoMind's clinical programme, with responsibility spanning on-site and remote visits, in-house operations support, and study oversight. This is a hands-on role with real breadth: you will be the primary relationship owner at investigator sites while also contributing to trial management and IRB coordination internally.
