Job Description The Consulting Pharmacist at Fuze Health supports the Central Operation telehealth healthcare team by providing remote outreach to Medicare members. This role focuses on ensuring member compliance with a new STARS measure and confirming successful emergency department discharge. Additional responsibilities may include closing other HEDIS measures, all while delivering compassionate, high-quality virtual care. Additionally, this role may at times work alongside the Pharmacy Operations team to support prescription fulfillment. Key Responsibilities: - Telehealth Patient Care: Provide remote patient care and support through telehealth platforms. - Virtual Assessments: Conduct virtual patient assessments, including reviewing medical histories and current symptoms. - Medical Documentation: Accurately assess and document patient conditions, histories, and care interactions. - Patient Education: Educate patients and their families on medical conditions, medication adherence, and self-care practices. - Care Plan Collaboration: Work closely with other healthcare providers to develop and implement individualized care plans. - Regulatory Compliance: Maintain accurate records and ensure adherence to healthcare regulations and privacy laws; maintain patient confidentiality with medication profiles. - Continuous Education: Stay up to date with medical advancements, telehealth protocols, and best practices through continuing education. - Compassionate Communication: Demonstrate empathy, professionalism, and effective communication in all patient interactions. - Pharmacy Support: Assist in the supervision of remote pharmacy technicians as needed; coordinate the provision of pharmaceutical services and delivery of medications and products to patients; ensure the security of legend drugs; verify the accuracy and appropriateness of prescriptions. Required Experience & Qualifications: - Active, clear, and unrestricted Pharmacist license in the State of Florida. - Registered Pharmacist in good standing with the Florida Board of Pharmacy. - Bachelor’s degree in Pharmacy or Doctor of Pharmacy (PharmD). - Minimum of 1 year of experience as a Registered Pharmacist. - Strong organizational and leadership skills to support clinical/pharmacy operations. - Strong computer skills and familiarity with electronic medical record (EMR) systems. - Ability to communicate clearly and effectively in both verbal and written forms. - Demonstrated ability to follow standard operating procedures and adapt in a fast-paced environment. - Strong customer service and interpersonal skills, especially in high-volume or high-stress scenarios. - Ability to work both independently and as part of a collaborative care team. Additional Information Physical requirements for this role include the ability to work at a computer terminal with monitor, keyboard and mouse for extended periods of time, periodically move away from the computer terminal for in-person meetings, stoop, bend, and reach for equipment and supplies, make frequent repetitive motions required to operate a computer that include the wrists, hands and fingers, and lift, carry, push, pull, and move objects up to 50 pounds. The role also requires the ability to effectively communicate through verbal interactions, discern auditory information, and visually perceive details to perform essential job functions.
