Role Description This is a fully remote position focused on supporting value-based care programs, primarily for Medicare and Medicare Advantage populations. The role involves: - Conducting chart reviews - Identifying care and quality gaps - Performing outreach activities to support gap closure initiatives A strong emphasis is placed on preventative care, health maintenance, and improving patient outcomes. The position requires an understanding of quality measures and population health management.
