Responsibilities - Virtual Nutrition Coaching: Conduct engaging one-on-one nutrition coaching sessions with patients via telehealth - Remote Patient Monitoring: Track, interpret, and respond to patient weight readings and other relevant health metrics - Personalized Care Planning: Develop customized meal and lifestyle plans, including support for patients using medications such as Ozempic - Patient Education: Provide guidance on nutrition, healthy habits, and long-term behavior change - Care Team Collaboration: Partner with physicians, pharmacists, and healthcare staff to ensure coordinated care delivery - Documentation & Reporting: Maintain accurate patient records in EMR systems and remote monitoring platforms
