Responsibilities and Impact will include: - Community Advocacy: Conduct high volume of inbound/outbound calls/SMS daily to Spanish and English-speaking members to educate them on vital preventative screenings. - Closing Care Gaps: Use your cultural competency to overcome barriers to care, helping members schedule appointments and navigate their benefits. - Data Integrity: Accurately document member interactions in our systems to ensure seamless care coordination. - Mission-Driven Performance: Meet and exceed outreach goals to ensure our members stay healthy and out of the hospital. Required Skills and Experience: - Customer-Service Background: 1+ years in a high-volume call center, retail, or hospitality environment where you learned to handle challenging conversations with grace. - Bilingual Excellence: Native-level or professional fluency in both Spanish and English in both written and oral communication (must be able to pass a language assessment). - Resilience & Agility: You thrive in high-volume environments and can pivot quickly between tasks and languages. - Technical Proficiency: Comfort navigating multiple computer screens, CRMs, and chat tools (like Slack) simultaneously. - Communication: A warm, empathetic phone presence with the ability to explain complex medical information in simple, culturally relevant terms. Desired Skills and Experience: - Experience in Healthcare: Previous experience with Medicare, Medicaid, or health insurance is a plus. - Remote Readiness: Previous experience working successfully in a remote environment. #LI-Remote Salary Range: $19-23 per hour The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. Role Description This position carries out the day-to-day activities of a physician referral service, including: - Referring incoming calls to appropriate physicians or clinics. - Notifying physicians when their patient has been admitted to the hospital. - Monitoring and updating multiple databases. This position is not responsible for providing patient care. Responsibilities - Facilitates access for patients, physicians, employees, and Outreach and Network Development to the system. - Answers inbound calls and refers callers to a specific physician or clinic based on the caller's stated medical issue(s). - Logs inbound calls and prepares quarterly reports that summarize referrals to specific physicians and clinics. - Generates daily reports that identify patients who were admitted the prior day. - Notifies the patient's physician (either internal or external to the organization) that their patient was admitted to the system and provides or helps gain access to pertinent medical information. - Takes ownership of solving problems or issues of importance to patients, physicians, and support staff. - Works to identify and notify patient physicians when little information is provided about the specific physician. - Maintains and updates several databases with accurate physician contact and demographic information, such as specialty areas, credentials, gender, and language(s) spoken in order to expedite referral services. - Assists other departments with collecting and entering doctor satisfaction surveys. - Based on call, initiates the process of receiving the patient into the system by creating a referral shell to send to the appropriate outpatient clinic for request for medical care.
