The content of this education should include, but not be limited to: pathophysiology of diabetes management (DM), nutrition management, pharmacological interventions, exercise, monitoring, prevention and management of acute and chronic complications, psychosocial adjustment, problem-solving skills, and stress management. Assesses readiness to change behavior and negotiates goals that promote a healthy lifestyle. • Develops an individualized MNT and DSMT complex plan of care appropriate to adolescent, adult and geriatric patient populations. Uses a variety of complex interventions and methods of behavioral management. • Collaborates with other members of the health care team to actively maintain the Diabetes Care and Resource Center as an American Diabetes Association Center of Excellence in providing Diabetes Self-Management Training (DSMT). • Researches trends in diabetes self-management training.
