Behavioral Health Outcomes Specialist II

Role Description This position is responsible for conceptualizing, recommending, developing, and implementing Behavioral Health Outcomes Based Care efforts in multiple CCO regions. Additionally, this position guides implementation and maintenance of plans and procedures through data informed technical assistance, coaching, learning events, and training to Behavioral Health plan employees and network providers in areas related to Outcomes Based Care, Quality Improvement, and behavioral health quality metrics across multiple CareOregon regions. This position leads projects and facilitates collaborative meetings, providing support to network leadership and care teams related to outcomes-based care in order to improve Quintuple Aim outcomes in Behavioral Health services. This requires a high degree of collaboration and influence with multiple internal stakeholders as well as external agencies and coordination with external clinical and operational leadership. Qualifications - Master’s degree in social work, counseling, public health or related field - Valid LCSW or LPC credential - OR - valid QMHP credential AND 3 years’ experience working in a behavioral health direct care setting - Minimum 5 years’ related experience (e.g., Behavioral Health Case Manager, Care Coordinator, Quality Improvement Specialist, etc.) that includes the following: - 2 years’ experience in behavioral health outcomes-based care - 2 years’ experience designing and delivering training and/or technical assistance to professional-level audiences - Experience in the Medicaid environment Requirements - Thorough and applied knowledge in areas related to behavioral health outcomes-based care - Applied knowledge in clinic operations and charting systems (e.g., EHR) - Thorough and applied knowledge with quality improvement tools and techniques in health care or related setting including quality measurement and improvement methods - Thorough and applied knowledge in Adult Learning Theory and training design and delivery - Ability to analyze and communicate complex behavioral health data for audiences with varied levels of data literacy - Ability to effectively coach and influence clinical and operational leadership and clinical teams of community groups that have distinct charters, missions, and goals using change management principles - Effective skills in managing internal and external group and organization dynamics, and facilitating consensus building - Skill in designing and delivering training and facilitating group discussions - Ability to operate with a high level of independence and autonomy and play a critical role in developing program goals, strategies, and policies - Ability to gain cooperation and support through discussion and persuasion - Ability to effectively analyze program goals and objectives to determine improved outcomes and take appropriate action - Identify program deficiencies or delivery problems, and develop corrective action strategies in support of goals - Ability to provide functional and technical supervision and input both inside and outside the organization to meet program goals, objectives, and outcomes - Strong ability to serve as professional representative of the organization in public and private forums - Advanced computer application skills including with MS Windows, Word, Excel, and Outlook with proficiency using Power Point, Access, Visio, and internet browsers/search functions - Strong capability in critical analysis, quantitative reasoning, and creative problem-solving skills - Excellent organizational skills and the ability to manage multiple tasks from multiple sources - Ability to work effectively with diverse individuals and groups - Ability to learn, focus, understand, and evaluate information and determine appropriate actions - Ability to accept direction and feedback, as well as tolerate and manage stress - Ability to see, read, and perform repetitive finger and wrist movement for at least 6 hours/day - Ability to hear and speak clearly for at least 3-6 hours/day Benefits - Competitive pay, bonus opportunity, and a comprehensive benefits package - Medical, dental, vision, life, AD&D, and disability insurance - Health savings account, flexible spending account(s), lifestyle spending account - Employee assistance program, wellness program, discounts, and multiple supplemental benefits (e.g., voluntary life, critical illness, accident, hospital indemnity, identity theft protection, pre-tax parking, pet insurance, 529 College Savings, etc.) - Strong retirement plan with employer contributions - PTO and Paid State Sick Time based on hours worked/scheduled hours - Paid holidays, volunteer time, jury duty, bereavement leave, and more, depending on eligibility

Title: Grants Manager, Community Health Location: New York Job Description: About RTW Foundation RTW Foundation (RTWF) was founded in 2018 as the philanthropic arm of RTW Investments, LP, a biotech investment firm. Through our three impact areas – rare disease, health equity, and STEM education – we power community initiatives and scientific research to improve the health of underserved populations. At RTW Foundation, we build partnerships with local organizations to advance health equity in New York City. One way that we support local health organizations is through our annual health equity grant cycle. We support partners beyond funding through co-created Days of Action, personalized capacity-building opportunities, and meaningful network connections. Our overall goal in community health is to strengthen the local health ecosystem in New York City. The Role The grants manager will facilitate the annual community health grant cycle, supporting the team and our partners to navigate through the full life cycle of a grant. Responsibilities will include: - Create all grant cycle materials including updated webpage, clear guidelines, LOI form, application form, reviewer materials, site visit guidelines, budget templates, fliers, social media content, etc. - Facilitate outreach to all potential applicants that align with set grant cycle focus and selection criteria. Research new community health organizations and create a landscape analysis of all key players in the NYC health ecosystem including CBOs, FQHCs, mid-sized nonprofits, large health systems, and peers funding organizations. - Field all applicant questions and requests. Support applicants and partners through the LOI and application process. Communicate criteria, scope, process, etc. Respond to applicant requests and questions in a timely, professional, and relational way. - Develop new grant cycle mechanisms like multi-year grants, streamlined renewal applications, more effective data management systems, etc. - Schedule and conduct partner site visits with key stakeholders. - Prepare and facilitate annual grant review committee meetings and review meetings with relevant Foundation leaders. Prepare recommendations for the Board of Directors. - Communicate approvals and denials with clear feedback, transparency, and kindness. - Communicate reporting expectations, follow up with partners, review all reports, summarize impact, and support partners with challenges that arise. - Maintain up to date comprehensive impact data. - Maintain up to date record of all key grant documents and information in Salesforce. - Effectively communicate partners’ impact. - Identify key disparities in health affecting underserved populations in New York City. In addition to the base salary, the role includes a discretionary incentive compensation program, and a full benefits package, including medical and life insurance, retirement plans, tax-free savings options, and access to other healthcare programs. Some community events will require a commitment outside of typical working hours, for example we co-create quarterly Days of Action with community partners on Saturdays.
