Role Description We are seeking a motivated and detail-oriented Medical Assistant to join our Chronic Care Management team. In this role, you will play a critical role in patient care by guiding patients through health assessments, reviewing medications and diagnoses, troubleshooting medical devices, and escalating clinical needs to nursing staff when appropriate. Key Responsibilities - Support CCM nursing staff by completing monthly patient assessments and conducting Chronic Care Planning Assessment (CCPA) calls, ensuring a positive, informative, and patient-centered experience. - Complete delegated clerical and administrative activities on behalf of nursing staff to support patient engagement, improve care coordination, and drive quality outcomes and overall patient experience. - Document patient interactions, assessments, and intake in the EHR. - Maintain compliance with company policies and applicable regulations. - Coordinate communication between patients, caregivers, nursing staff, and providers to ensure continuity of care and timely follow-up on patient needs. - Perform other duties, as assigned.
