You’ll spend your days connecting with families, helping them navigate Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT), and making sure those you serve can live safe, healthy, and independent lives. Qualifications - At least 2 years of experience supporting individuals with disabilities or the elderly. - No degree required (we care more about heart and experience). - Tech-savvy enough for email, docs, spreadsheets, portal navigation and video calls. - A valid driver’s license & reliable vehicle (no client transport). - Great communication—both written and spoken. - Able to pass a background check + provide 2 professional references. - Bonus points if you’ve got Person-Centered Thinking/Planning training. • Responsible for provision of telephone-based assessment, triage, consultation, counseling, and informational services, delivered in a call center environment using a variety of advanced clinical assessments, motivational Interviewing, and solution focused/short-term problem resolution • Provides risk, substance abuse and mental health assessments to develop immediate action/safety plans based on level of urgency • Understands various levels of care (mental health and substance use) to aid in appropriate recommendation of care based on the assessment of need • Provides information to members regarding mental health, substance abuse, community treatment, and related mental health programs to assist members with linkage to resources • Collaborates with professional teams to support a positive clinical outcome and continuity of care for all members

• Deliver direct client counseling services to families, couples, and children • Utilize therapeutic approaches tailored to the needs of the client • Attend to relational ethics while collaborating with interdisciplinary team and families • Participate in family care meetings with other providers • Comply with clinic policies and regulations fostering adherence to standards of care • Maintain documentation and communication in line with legal, ethical, and company standards
