Pay Range The typical pay range for this role is: $37.66 - $99.14 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. Role Description The Chronic Care Management (CCM) Assistant – Clinical Check Specialist plays a critical role in Carda Health's private partnership programs, supporting the delivery of safe, personalized care through thorough clinical checks, data accuracy, and proactive patient assessment.
