• Welcome new patients and confirm delivery of care packages and equipment • Facilitate initial telehealth visits and conduct care package walkthroughs • Guide patients through device setup and navigation of the Carda Health app • Provide clear instructions and ongoing support to ensure patient comfort with technology and program participation • Manage check-in and check-out processes for telehealth appointments • Coordinate with providers and internal teams to ensure smooth appointment flow and follow-up • Serve as part of the frontline support team for patients, answering questions and resolving issues with empathy and efficiency • Keep patients engaged, motivated, and informed throughout their care journey • Accurately document all patient interactions in Salesforce and related systems • Track care package deliveries, referrals, and enrollment statuses to ensure no patient is left behind • Collaborate with clinical, growth, and operations teams to address patient needs and improve workflows

• Evaluate prescriptions and treat cardiac/pulmonary rehab patients according to our clinical protocols. • Manage a caseload of patients in our Phase 3 maintenance program, driving adherence, patient satisfaction, and continued progress for patients who have graduated from our Phase 2 program. • Be responsible for responding to alerts and improving adherence to patients participation in vitals monitoring as part of our Remote Patient Monitoring (RPM) program. • Review SpO2 readings from patients participating in our Overnight Pulse Oximetry Monitoring (ONPO) program. • Complete necessary documentation and billing claims for Phase 3 rehab sessions, RPM encounters, and ONPO reports. • Wear many hats: anything from designing patients’ exercise programming to responding to their technical issues. • Work directly with founders to innovate and improve our cutting edge digital care delivery model. • Work collaboratively with our technology team to support a smooth experience for our patients and clients. • Be willing to do whatever it takes to ensure a great, engaging rehab experience for our patients. • Take pride in delivering high quality care for patients going through our rehab program. • Provide support to our patients, empowering them to be accountable and take ownership of their rehabilitation.

• Oversee a patient caseload to complete the setup and removal of ECG recorders • Conduct patient screenings for ECG program (review qualifications, eligibility, etc) • Participate in ECG patient enrollment - to include discussions with patients over the phone or in sessions • Review, analyze, and interpret ECG rhythms • Communicate ECG findings with internal and external physicians for review • Complete necessary documentation and billing claims for ECG monitoring • Wear many hats: anything from designing patients’ exercise programming to responding to their technical issues • Work directly with founders to innovate and improve our cutting edge digital care delivery model • Work collaboratively with our technology team to support a smooth experience for our patients and clients • Be willing to do whatever it takes to ensure a great, engaging rehab experience for our patients • Take pride in delivering high quality care for patients going through our rehab program • Provide excellent support to our patients, empowering them to be accountable and take ownership of their rehabilitation

• Own end-to-end clinical operations across all Carda programs • Build, manage, and mentor a high-performing remote clinical team • Develop and enforce clinical SOPs, compliance frameworks, and accreditation standards • Partner closely with Growth and Partnerships to support program launches • Collaborate with Product and Engineering on clinical needs • Define, track, and report on clinical KPIs • Serve as a key external-facing clinical voice with partners
