What You Will Do - Essential Functions The Care Coordinator supports the chronic care management team by coordinating non‑clinical care activities, providing essential administrative support, and ensuring accurate documentation and communication to enable efficient, high‑quality, and patient‑centered care delivery. - Non‑Clinical Care Coordination & Patient Support - Coordinate non‑clinical components of patient care plans, including referrals, DME orders, appointment scheduling, and follow‑up activities. - Serve as a primary resource for patients and families for routine questions, ensuring timely escalation of clinical issues to the appropriate care team members. - Administrative & Operational Support for Care Delivery - Act as a first point of contact by managing incoming phone calls, faxes, and correspondence, and routing inquiries appropriately. - Provide administrative and workflow support to the care team and organization, including special projects and tasks that enhance program effectiveness and patient outcomes. - Data, Documentation & Records Management - Facilitate the receipt, organization, and reconciliation of external medical records critical to patient care. - Maintain accurate, timely data entry and documentation within the electronic health record (EHR) to support care coordination, compliance, and reporting requirements. Who You Are Required Qualifications: - Minimum of a High School diploma - Completion of a Medical Terminology course preferred - Medical office experience preferred - Five years of work experience preferred - Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.
