Job Duties Conducts patient contact and appointments to provide education, support, and resources - Ability to work with patients via direct contact and telephonically - Ability to make prompt and accurate clinical and psychosocial assessments of mental health conditions, determine patient needs and barriers, identify the appropriate provider to meet the need, and develop creative solutions to patient barriers - Develop, coach, and support effective patient/family relationships for successful patient navigation Resource to providers, clinical staff, and care coordinators - Ability to work effectively as a team interacting with patients, primary care providers, site staff, and primary care team - Act as healthcare and community resource liaison/advocate for the patient/family - High level of comfort working with primary care providers using a consultative style. Administrative duties to include: - Facilitate appropriate patient referrals to other healthcare providers and community resources - Act as navigator for patient referrals with healthcare providers and community resources (i.e. complete Medicaid/Medicare forms) - Develop and maintain relationship with healthcare and community resource providers - Develop and maintain healthcare and community resource/service guide (i.e. housing needs, transportation, financial assistance, meals) Additional Job Description Physical Requirements: Ability to perform job functions in an upright position.
