Certified Coder – Orthopedic Surgery

• Review medical record documentation to determine appropriate billing codes and necessary documentation • Codes evaluation and management to appropriate CPT code and codes diagnosis to appropriate ICD-10 code • Meet with physicians to review documentation, resolve coding and secure signature of all unsigned dates of service • Act as the lead person and assist coders with medical terminology and policy interpretation • Assist with efforts to increase physician awareness of documentation requirements • Prepare case reports and initiate follow-up for billing process • Perform other duties as assigned

Job Requirements

  • Must have one of the following coding credentials: AHIMA (CCA, CCS, or CCS-P); AAPC (CPC, CPC-A, CPC-H, CPC-H-A, or one of the AAPC specialty-specific coding credentials)
  • Previous coding experience or experience equivalent to an associate’s degree in a related field preferred
  • Knowledge of ICD-10 and CPT coding preferred

Nemours Children's Health is hiring a remote Professional Fee Abstractor. The Professional Fee Abstractor is responsible for 100% charge capture by reviewing physician documentation and properly coding all professional services performed utilizing appropriate ICD-10-CM, CPT-4, HCPCS codes and modifiers. Daily review of EPIC Charge Review Work queues is essential. Participation in physician training and education is also required. Maintaining yearly certification as a CPC, CCS-P, RHIA, or RHIT is required. This is a remote position. Essential Functions: 1.Ensure that charges for all applicable patient encounters of the Nemours Children’s Clinical Practice are abstracted in a timely manner. Utilization of hospital systems will be used to indicate respective patients. 2.Abstracted charges will be reported via EPIC EHR, external interfaces or other approved methods 3.Communicate in a timely manner with physicians, managers, and peers orally or in writing, regarding medical record documentation guidelines and regulations, and respond to all inquiries that concern the proper documentation of diagnostic and procedural services. 4.Maintain thorough knowledge of coding, and documentation principles. Also maintain knowledge of assigned medical specialties. 5.Attend in-services and seminars pertinent to assigned medical specialties and documentation requirements 6.Review and maintain current knowledge of changes in coding guidelines and regulations. 7.Meets at least quarterly to discuss new coding/ documentation guidelines with management and team 8. Reports significant changes in documentation trends that affect assigned levels of service or procedures being billed 9. Adhere to the Nemours Standards of Behavior 10. Summary/Objective Under limited supervision the Coder Physician reviews medical records and performs coding on all diagnoses, procedures, DRG/APC, and charge codes. The Coder Physician uses the most accurate codes for reimbursement purposes, research, epidemiology, statistical analysis outcomes, financial and strategic planning, evaluation of quality of care, and communication to support the patient’s treatment. The Coder Physician will be charged with maintaining the confidentiality of patient records and procedures. Essential Job Functions - Responsible for abstracting, coding, sequencing and interpreting the clinical information from inpatient, outpatient, emergency department, pro fee, and clinical medical records. - Responsible for the assignment of correct principal diagnoses, secondary diagnoses and principal procedure and secondary procedure codes with attention to accurate sequencing. - Utilizes technical coding principals and DRG/APC reimbursement expertise to assign appropriate codes. - Abstracts and codes pertinent medical data into multiple software programs and/or encoders. Follows official coding guidelines to review and analyze health records. - Maintains compliance with both external regulatory and accreditation requirements, and with State and Federal regulations. - Extracts pertinent data from the patient’s health record and determines appropriate coding for reports and billing documents. - Identifies codes for reporting medical services, procedures performed by physicians. Enters codes into various computer systems dependent upon the various clients. - Track and document productivity in specified systems, maintain productivity levels as defined by the client. - Maintain 95% quality rating - Perform duties in compliance with Company’s policies and procedures, including but not limited to those related to HIPAA and compliance.
