Make a Difference—Even From Home We are seeking an experienced and empathetic Hospice Registered Nurse (RN) to join our team in a remote triage-only role. Key Responsibilities - Provide after-hours triage and clinical support by phone for hospice patients and families. - Assess symptoms, provide guidance, and determine if an in-person visit is necessary. - Communicate effectively with the interdisciplinary hospice team to ensure coordinated care. - Offer compassionate and calm support to patients and families during emotionally sensitive situations. - Document all patient interactions promptly and accurately in the EMR system. - Participate in on-call rotation and maintain professional responsiveness throughout shift hours. Note: This is a triage-only position. No scheduled in-person visits are required. Qualifications - Active Washington State RN license (or compact equivalent). - Hospice triage experience required. - Must be available 5 pm to 8 am PST, 7 days on with 7 days off - Strong clinical assessment, communication, and critical thinking skills. - Ability to work independently and maintain composure during high-stress situations. - Must have HCHB experience. - Reliable home internet and private workspace for remote triage. Role Description The Registered Nurse (RN) – Enhanced Care Management (ECM) will join Zócalo Health during a period of rapid growth and increasing operational complexity.
