Associate Manager, Engage Pharmacist

• Develop, implement, and evaluate population health initiatives centered around medication management, adherence, and optimizing therapeutic outcomes. • Utilize health data analytics to identify trends, gaps in care, and opportunities for intervention. • Collaborate with healthcare providers to ensure appropriate medication selection, dosing, and monitoring. • Design and implement programs to educate patients about their medications. • Work closely with healthcare teams to coordinate care plans and interventions. • Participate in quality improvement projects aimed at enhancing medication-related outcomes. • Provide patient care in an appointment-based model for both dispensing and non-dispensing patients. • Stay updated with pharmaceutical regulations, ensuring compliance. • Responsible for hiring, training, and managing a team of Pharmacy Care Coordinators.
Job Requirements
- Doctor of Pharmacy (PharmD) degree from an accredited institution.
- Active pharmacist licensure in the state or region of practice.
- Advanced training or certification in population health management or a related field (preferred).
- Strong analytical and data interpretation skills.
- Experience in pharmacy practice, preferably in a population health or managed care setting.
- Excellent communication, collaboration, and leadership abilities.
- Knowledge of healthcare regulations, policies, and quality improvement methodologies.
