• Market Physician Executive (MPE) are mission driven physician leaders who are dedicated to improving the well-being, quality of life, and health outcomes for our patients • The MPE will lead our in-home multi-specialty polychronic care model in an assigned market • Collaborate with Monogram Health’s Multi-Specialty Platform to leverage employed specialists to deliver in-home specialty care • Reporting to the Region President, the Market Physician Executive (MPE) oversees the daily clinical and business operations through delivery of direct patient care, care management services, social worker support and pharmacy services within the market • Provide clinical guidance and direction to Market teams to drive Population Health Management activities • Engage with patients on treatment plans, community provider collaboration, and direct evidence-based care pathways • Conduct patient reviews to target high utilizers, high risk and high opportunity patients and patient cohorts

Job Requirements

  • Must be willing and able to obtain hospital privileges at required facilities
  • Demonstrated experience applying evidence based clinical criteria
  • Experience in renal care and geriatrics
  • Strong management and communication skills
  • Active, unrestricted state medical license required in each state within the market
  • Experience with high need Medicare Advantage and managed Medicaid populations
  • Experience with NCQA, HEDIS, Medicaid, Medicare, quality improvement, medical utilization management, and risk adjustment
  • Current state medical license without restrictions to practice and free of sanctions from Medicaid or Medicare
  • Willingness to become licensed in multiple states
  • MD (Medical Doctor) or DO degree from an accredited medical school
  • BC or BE in an ACGME approved specialty such as Nephrology, Internal Medicine, Family Practice, Emergency Medicine, Critical Care, Cardiology, Endocrinology, Hepatology, or Geriatrics

The Philanthropy unit, part of the ER department, leads on the organization’s work with High-Net-Worth Individuals. Positioned within the USA Philanthropy department, the Major Giving West team has a goal of improving private individual support for the IRC by engaging, cultivating, soliciting, and stewarding significant, multi-year commitments from high-net-worth individual supporters based in the western United States (west of the Mississippi River). The Director, Major Giving West (Southwest) will lead the IRC’s Major Giving program in the southwestern region of the US (including the greater Los Angeles region, Southern California, New Mexico, Arizona, Texas, and Utah) by leading a team of fundraisers in managing and developing regional markets with portfolios of donors, prospective donors, and suspects with wealth capacity ratings of at least $100k and up to $2.5M in addition to their own portfolio with capacity ratings of at least $500k and up to $2.5M. The Director will craft and implement fundraising strategies for her/his/their portfolio and will mentor and guide the team to optimize the size and impact of gifts to IRC. They will encourage fundraisers to function as relationship managers, using donor-centric fundraising practices, and deploying IRC leaders throughout donor life cycle to qualify, cultivate, solicit, and steward five and six-figure gifts. The Director serves a critical role in helping to develop and implement strategies that are informed by bold revenue targets. She/he/they will be expected to understand the needs and performance of donors and prospects, communicate and collaborate superbly with colleagues, think strategically, and offer creative solutions to continually grow and improve donor relationships. Major Responsibilities Fundraising · Manage a small portfolio of Major Giving capacity donors, prospects, and suspects that results in retaining donors and realizing ever-increasing size gifts at the six and seven figure level. · Craft and implement fundraising strategies for Major Giving portfolio to optimize the size and impact of gifts to IRC. · Travel and meet with a portfolio of donors and prospects across the Southwestern US region. · Achieve goals and metrics related to donor visits, solicitations, gifts and pledges closed, and other critical key performance indicators.
