• Lead individual therapy sessions in a telehealth setting • Participate in supervision or consultation to enhance clinical skills and receive support as needed • Work remotely in a fully flexible, but supportive environment

• Provide individual tele-therapy to patients requesting regular sessions using FamilyWell’s virtual platform. • Conduct one-on-one psychological intakes for referred FamilyWell program patients, gathering comprehensive assessments to inform treatment planning. • Document patient progress comprehensively, sharing treatment recommendations with medical providers, psychiatric consultants, and other treating professionals as needed. • Facilitate treatment plan modifications for patients needing more intensive or specialized mental health care, ensuring timely and appropriate interventions. • Develop and deliver Care Plans post-session, offering patients actionable insights and follow-up steps.

• Assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s); • Provides patient, caregiver, and family counseling; • Provides patient and caregiver education; • Prepares clinical notes, and progress summaries based on the attainment of goals; • Communicates with the physician who is responsible for the home health plan of care; • Provides physical therapy services to patients according to a written physician's plan of care; • Evaluates home environment; • Assesses for muscle strength, mobility, gait, ROM—potential for rehab; • Directing physical therapy treatment; • Instructing patients and families/caregivers in the use and care of therapeutic appliances; • Determining priority needs for physical therapy; • Reporting to physician patient’s reaction to treatment or changes in condition; • Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy; • May train patient in the use of prosthetic device; • Identifies patient and family/caregiver needs for other home health services and refers as necessary; • Participates in discharge planning for patient; • Provides in-service education programs for nursing organization personnel as needed; • Participates in peer consultation process; • Supervises Physical Therapy Assistants according to organization policy and state regulations.

• Offer therapy services for individuals, families, and groups as well as Telehealth • Evaluate mental health diagnosis, create, and implement a treatment plan, and complete ongoing documentation including session progress notes, continual assessments, treatment plan reviews, and any other contact notes according to company guidelines and protocols • For Full-Time status, clinicians must maintain a caseload of a minimum of 25 client visits completed per week • Provide excellent customer service for clients and collaborate with a dynamic team to further the mission of filling gaps in our community • Utilize creativity in interventions to help clients achieve and exceed goals • Be an advocate for your client by coordinating services with case managers, families, work personnel, medical personnel, other Ellie staff, and school staff as needed • Attend and participate in all clinical staff meetings and training
