Job Requirements

Role Description L’ASS, assujetti au secret professionnel, assure le service social auprès des malades hospitalisés, services médecine physique - réadaptation relevant du CTAS de Saint-Germain-en-Laye à l’antenne d’action sociale Percy Patients. - Mettre en œuvre les dispositifs d’action sociale du ministère en complément des dispositifs nationaux et faciliter l’accès aux droits sociaux. - Assurer le soutien social des ressortissants du ministère des armées gravement blessés en service et notamment sur les territoires d’opérations extérieurs (TOE) ainsi que l’aide à leurs familles en mobilisant les dispositifs institutionnels et la procédure évènements grave. - Participer à l’élaboration du projet thérapeutique et mettre en œuvre un plan d’aide adapté en liaison avec les équipes médicales et paramédicales. - Apporter son expertise au commandement local sur des situations individuelles et collectives, dans le respect des règles de la profession. - Intervenir sur les évènements graves selon les procédures. - Renseigner les outils de recueil d’indicateurs des données sociales, en particulier le système informatique de l’action social (SIAS). - Participer à l’organisation des sorties des patients hospitalisés : orientations vers soins de suite et réadaptation, vers l’unité de soins palliatifs, l’hospitalisation à domicile, retours à domicile. - Assurer l’interface entre les services, les patients et les familles des patients hospitalisés. Qualifications - Titulaire du diplôme d’état d’assistant de service social (DEASS). - Maîtrise des dispositifs de solidarité de droit commun. - Compétences en méthodologies d’interventions en service social et d’élaboration des écrits professionnels. - Esprit d’analyse et de synthèse. - Capacité à apporter conseil et aide à la décision aux autorités. - Appétence pour la conduite de projets et goût pour le travail collaboratif. - Autonomie, disponibilité, qualités relationnelles, d’adaptabilité et de réactivité. - Expérience confirmée d’assistant de service social en métropole. Requirements - Possibilité d’exercer des missions sur certains postes localisés en Outre-Mer ou à l’étranger. The Teacher is a highly qualified, state certified educator responsible for delivering specific course content in an online environment. Teachers provide instruction, support, and guidance, manage the learning process, and focus on students’ individual needs.
