PDMI is looking to add a 340B Account Coordinator to our team! As a 340B Account Coordinator, you will work closely with 340B Account Managers to service new and existing clients within the 340B space by developing expertise in this particular line of business. Since 1984, PDMI has provided pharmacy data processing and other flexible, scalable solutions to help our clients meet their business objectives. We offer transparent, pass-through pharmacy processing and other services for private label Pharmacy Benefit Managers (PBMs), vertically integrated health plans and hospital systems. In addition to Pharmacy Benefit Administrative Services, we offer 340B Administration, Hospice and Long-Term Care Services. Responsibilities: - Client Onboarding and Program Setup: Assist with obtaining client data, reviewing implementation forms, setting up program access, and configuring reporting parameters. - Data and Feed Management: Monitor, verify, and coordinate client data feeds, including SFTP setup and ensuring accuracy and completeness. - Training and Support: Provide training to clients and pharmacies on proprietary reports and point-of-sale programs; act as backup support for Account Managers. - Operational Coordination: Collaborate with IT Services, manage operations support requests, and participate in team/client meetings. - Documentation and Compliance: Maintain detailed client files, verify program eligibility, and ensure documentation for new client setups and HRSA requirements. - Issue Resolution and Escalation: Handle claims resolution, address client inquiries, and escalate complex issues as needed. - Continuous Development and Cross-Training: Stay updated on 340B program education, support team absences, and assist in product enhancements. Position Responsibilities - Find community-based support to meet enrollees' housing, transportation, food insecurity, education, and employment needs, in addition to other SDOH needs. - Support Care Managers and Community Health Workers to find appropriate community housing resources and to address additional SDOH needs. - Liaise among Humana Care Managers, providers, and community-based organizations to coordinate referrals for enrollees to community-based services and programs and to foster integrated efforts among all parties. - Handle calls from Enrollee Services for callers who require resources and who are not participating in Care Management; can refer enrollee to Care Management after initial resource assistance. - Assist with health screening process, unable to reach efforts, conduct non-clinical surveys, and offer Care Management services, as needed. - Serve as a key knowledge source for community services and information for enrollees and other Humana associates. - Implement community assessments to identify community resource gaps. - Monitor enrollee compliance with their care plan and provide motivational interviewing to support medication and treatment adherence. - Support enrollees' self-management skills through the provision of culturally appropriate health education and health coaching - Attend Humana community events to connect with enrollees and provide education on care management services. - Build and maintain relationships with providers and community resources to support enrollee identification and referrals.
