Job Requirements

  • Registered Nurse licensure in Michigan
  • Two or more years of clinical case management, care management, and/or care coordination experience, preferably in an ambulatory/outpatient setting
  • Caring, professional demeanor and excellent customer service skills
  • Excellent critical thinking, assessment, and problem-solving skills
  • Ability to work independently with minimal supervision
  • Experience working in Electronic Medical Records, especially Epic
  • Ensure protection of patients’ privacy and Protected Health Information (PHI) in a home office environment
  • Reliable wi-fi.
