Job Requirements
- Registered Nurse licensure in Michigan
- Two or more years of clinical case management, care management, and/or care coordination experience, preferably in an ambulatory/outpatient setting
- Caring, professional demeanor and excellent customer service skills
- Excellent critical thinking, assessment, and problem-solving skills
- Ability to work independently with minimal supervision
- Experience working in Electronic Medical Records, especially Epic
- Ensure protection of patients’ privacy and Protected Health Information (PHI) in a home office environment
