• Provide motivational support and educational communications to 9amHealth members that are managing obesity, diabetes, hypertension, high cholesterol, and weight concerns via zoom. • Use motivational interviewing, and coaching techniques to support lasting behavior changes in areas like nutrition, sleep hygiene, movement, and stress management using standards of care aligned with NBC-HWC. • Maintain an up-to-date calendar to support member visit scheduling. • Utilize member data to personalize sessions and support members in achieving their health goals. • Complete accurate documentation of all member visits and interactions in the electronic medical record (EMR). • Reinforce nutrition and lifestyle plans developed by 9amHealth RDs, tailoring guidance to each member’s goals and preferences. • Escalate member concerns and help deliver an exceptional care experience. • Deliver empathetic, inclusive care that respects and reflects the diverse backgrounds, identities, and experiences of all members.
Job Requirements
- Active certification in health coaching or related fields. NBC-HWC is required.
- Expertise in chronic conditions like diabetes, hypertension, high cholesterol, and obesity
- 5+ years of direct client care, ideally in clinical or telehealth settings focused on chronic disease coaching
- Comfort with digital tools, including EMRs and coaching platforms
- Strong relationship-building skills with compassionate, personalized communication
- Self-directed and reliable, with a strong sense of ownership and accountability in delivering high-quality care independently
- Virtual care experience, including chat, phone, and video
- Dedicated, private workspace with reliable high-speed internet for confidential care delivery
