The Outreach Care Specialist I is responsible for ensuring that all day to day activities remain in operation for the benefit of the members with primary support to the case managers. How you will make an impact: - Ensures that all reports are completed timely, regulations are met as well as timeliness standards for follow up. - Mandatory monthly reports and meetings. - Ensure the Plan is in compliance with the state metrics. - Identifies opportunities that impact quality goals and recommends process improvements. - Establishes and maintains relationships with agencies identified in appropriate contract. - Identifies barriers to plan compliance and coordinates resolutions. - Identifies opportunities that impact quality goals and recommends process improvements. - Prepares reports to document case and compliance updates. Minimum Requirements: - Requires a H.S. diploma or equivalent and a minimum of 1 year related experience; or any combination of education and experience which would provide an equivalent background. Preferred Skills, Capabilities & Experiences: - BS/BA degree in a related field (Human Services, Social Work, Psychology, Substance Abuse, etc.) preferred. - Non- licensed Case Manager experience, where one is able to assist in engaging members in services and share resources. - For URAC accredited areas, the following professional competencies apply: Associates in this role are expected to have strong oral, written and interpersonal communication skills, problem-solving skills, facilitation skills, and analytical skills. Required Qualifications • At least 6 years of provider services experience, including experience supporting individual/group providers, hospitals, integrated delivery systems, and ancillary providers with Medicaid, Medicare, and or Marketplace products, or equivalent combination of relevant education and experience. • Strong understanding of the health care delivery system, including government-sponsored health plans. • Experience with various managed health care provider compensation methodologies, primarily across Medicaid and Medicare lines of business, including: fee-for service (FFS), capitation and various forms of risk, ASO, etc. • Previous experience with community agencies and providers. • Strong organizational skills and attention to detail. • Ability to manage multiple tasks and deadlines effectively. • Strong interpersonal skills, including ability to interface with providers and medical office staff. • Experience with preparing and presenting formal presentations. • Project management experience. • Ability to work in a cross-functional highly matrixed organization. • Strong verbal and written communication skills. • Microsoft Office suite and applicable software programs proficiency.
