Responsibilities - Deliver comprehensive clinical care through both text-based (asynchronous) and video-based (synchronous) consultations, ensuring the wellbeing of the patients - Empower patients with knowledge by educating them on suitable treatments and personalized care plans for their specific health needs - Oversee patient follow-ups for ongoing care programs, ensuring continuity and effectiveness of treatment - Prescribe medication and provide timely refills as needed, ensuring patients have access to appropriate medications - Maintain accurate and timely electronic health records (EHR) in compliance with all relevant regulations - Adhere to all ethical and professional standards of practice, including patient confidentiality and informed consent - Participate in team meetings, chart audits, and continuing education activities - Stay informed of current research, best practices, and advancements in healthcare - Provide medication evaluation and management to patients within the LifeMD family of services including women's health, men's health, urgent care, behavioral health and other conditions, as needed by management

Job Requirements

  • Basic Qualifications:
  • Master's or Doctoral degree from an accredited nursing program with a specialization in Family Practice, or similar
  • Current, unencumbered FNP license in multiple states (additional state licenses may be required based on patient location)
  • Board certification in Family Nurse Practitioner (FNP) through the ANCC
  • 2+ years of experience as an NP, with demonstrated experience in telehealth
  • Primary care experience with the ability to diagnose and treat both acute and chronic care conditions
  • Familiarity with telehealth platforms and electronic health record (EHR) systems
  • Preferred Qualifications:
  • 20+ active state medical licenses, in good standing
  • Strong communication skills for both live video and asynchronous messaging
  • Excellent critical thinking, problem-solving, and decision-making abilities
  • Ability to work independently and as part of a collaborative team
  • Commitment to providing patient-centered, compassionate care

Knowledge, Skills & Abilities - Active and unrestricted LPN/LVN license in MA with willingness to obtain additional licensure as needed. - Strong understanding of scope of practice and clinical decision-making for remote patient monitoring. - Experience in primary care, chronic disease management, or population health preferred. - Proficiency with EMR/EHR systems and comfort using digital tools to manage patient data. - Excellent verbal and written communication skills, with empathy and professionalism in every patient interaction. - Strong organizational and documentation skills, with attention to detail and accuracy. - Collaborative mindset and ability to thrive in a tech-forward, fast-paced environment. Preferred Experience - Minimum 3 years of practical nursing experience in a primary care, community health, or telehealth setting. - Experience addressing care coordination, medication adherence, and social determinants of health. - Exposure to AI- or technology-supported care delivery models is a plus.
