Patient Outreach Coach
Better Health Group
• Conduct initial assessments to understand clients' health goals, concerns, and challenges • Develop personalized health plans based on the client's needs, incorporating nutrition, exercise, stress management, and other relevant factors • Provide ongoing coaching and support to clients, helping them stay motivated and accountable • Educate clients on healthy lifestyle choices, including proper nutrition, exercise routines, and stress-reduction techniques • Monitor clients' progress and make adjustments to their health plans as needed • Collaborate with other healthcare professionals, such as doctors and nutritionists, to ensure comprehensive care for clients • Stay up-to-date with the latest research and trends in health and wellness • Maintain accurate and confidential client records • Additional duties as assigned
Job Requirements
- Bachelor's degree in a related field (e.g., health promotion, nutrition, psychology) is required
- Certification as a Health Coach from a recognized program is required
- 1 + year of experience working as a Health Coach
- Proven experience in a leadership role within a health coaching or wellness program
- Strong knowledge of health and wellness principles, behavior change theories, and evidence-based coaching techniques
- Proficiency in using technology and software applications relevant to health coaching
- Demonstrated commitment to continuous learning and professional development in the field of health coaching
- Proficient with Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint) for real-time collaboration
