The Community Health Worker is responsible for assessment, planning and implementing care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care. They also: - Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services. - Manage the care plan throughout the continuum of care as a single point of contact for the member. - Act with Medicaid members to ensure appropriate care is accessed. - Provide home and social assessments and member education. - Address social determinants of health such as transportation, housing, and food access. This position is full time (40 hours/week), Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00 am - 5:00 pm CST. It may be necessary, given the business need, to work occasional overtime. This position is a field-based position with a home-based office. You will work from home when not in the field. Local travel up to 25% and mileage is reimbursed at current government rate. We offer 3 - 4 weeks of paid training. The hours during training will be 8:00 am - 5:00 pm, Monday - Friday. Attendance is 100% required. Training will be conducted virtually from your home. Primary Responsibilities: - Create a positive experience and relationship with the member. - Help member set goals and develop a care plan to achieve those goals. - Proactively engage the member to manage their own health and healthcare. - Support the member to improve their wellbeing by staying out of the hospital and attending regular visits to their primary physician. - As needed, help the member engage with mental health and substance use treatment. - Support the member to ensure pick-up of their prescriptions. - Provide member education on community resources and benefits. - Conduct post discharge activities from hospital facility and support connection to social services. - Support member to engage in work or volunteer activities, if desired, and develop stronger social supports through deeper connections with friends, family, and their community. - Partner with care team (community, providers, internal staff). - Knowledge and continued learning of community cultures and values. - May conduct Health Risk Assessments (HRA) if needed. - Perform other duties as assigned. Qualifications - High School Diploma / GED. - Must be 18 years of age OR older. - 6+ months of Field-based experience of community advocacy. - Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI). - Advanced computer proficiency experience including Microsoft Word, Microsoft Excel, and Microsoft Outlook. - Ability to travel locally approximately 25% of the time up to 100 miles round trip. - Reliable transportation, that will enable you to travel to client and/or patient sites within a designated area, with a current and non-restricted state of Minnesota Driver’s License and State-required insurance. - Familiarity with the resources available in the community. - Ability to work full time (40 hours/week), Monday - Friday.
