Role Description The Transition Operations Manager is critical to Monarch’s success. They are responsible for leading the transition process end-to-end from onboarding new physicians from the moment they sign with Monarch through the successful launch of their practice. This role serves as the primary point of contact for physicians throughout the transition, ensuring that each launch is smooth, on time, and aligned with Monarch’s operational and growth goals. Transition Managers lead panel development and marketing efforts, coordinate practice setup, and manage all the moving pieces required to bring a new practice live. Success in this role requires strong relationship management, project management, and problem-solving skills. Every transition is unique and the Transition Manager must be able to adapt, prioritize effectively, and coordinate across teams to address the specific needs of each physician. Key Responsibilities - Physician Relationship Management - Serve as the primary point of contact for physicians throughout the transition process. - Build trusted relationships with physicians and guide them through each step of their practice launch. - Provide proactive communication, updates, and support to ensure physicians feel confident and informed throughout the transition. - Identify risks, challenges, or concerns early and work collaboratively to resolve them. - Transition & Launch Project Management - Own the end-to-end transition timeline from physician signing to practice go-live. - Manage the many parallel workstreams required to launch a new practice, including: - Real estate and office setup - Equipment and supplies procurement - EMR transition and technical setup - Hiring and onboarding of practice staff - Coordinate closely with cross-functional partners across operations, marketing, recruiting, IT, and finance. - Track milestones, manage dependencies, and ensure transitions remain on schedule. - Panel Development & Marketing Coordination - Lead the panel development strategy to ensure physicians meet patient growth targets ahead of launch. - Partner with marketing and sales teams to drive patient acquisition and awareness. - Monitor progress toward panel targets and adjust strategies as needed. - Cross-Functional Coordination - Act as the central hub of communication across all teams involved in a transition. - Provide regular status updates on transition progress, milestones, and risks. - Escalate issues when needed and drive resolution with appropriate stakeholders. - Problem Solving & Process Adaptation - Navigate the unique circumstances of each physician transition with creativity and flexibility. - Adjust processes, timelines, and approaches based on physician needs and operational realities. - Continuously identify opportunities to improve transition processes and physician experience. You will collaborate with a team of physicians, Advanced Practice Providers (APPs), and Interdisciplinary Team (IDT) members. Nurse Care Managers at Evergreen often focus on patients targeted for specific programs such as Chronic Complex Care Management, Compassionate Care Management, Post Acute Care, Transitions of Care, and CKD Management. While our Nurse Care Manager positions are fully remote, this specific position will support patients in the Eastern Time Zone and must be able to work 8:30a - 5p EST.
