• Work with stakeholders to understand business needs and design technical solutions that will meet quality needs for security, scalability, reliability, usability, and accessibility. • Collaborate with team to develop, test, and implement technical solutions using configurations and customizations in enterprise software systems in an agile development process. • Follow development standards for solution design, development, testing, implementation, and documentation and work with team to continually refine and improve those standards. • Perform testing for solutions to ensure releases meet standards for efficiency, quality, security, and reliability. • Provide maintenance and technical support for enterprise applications in accordance with organizational standards and service-level agreements. • Keep up with industry trends and learn new technologies as needed to meet company needs.

Job Requirements

  • Bachelor’s degree in IT discipline or equivalent work experience.
  • 2+ years of experience in designing technical solutions and administration of enterprise applications or a similar role.
  • Flexibility and ability to adapt to changing conditions.
  • Strong attention to detail.
  • Ability to work well independently and as part of a team.
  • Excellent written, verbal, and interpersonal communications skills.

If you are in or within a 60 mile radius driving distance from Dallas County, TX to support the following zip code areas 75216, 75237, 75211, 75062, 75061, 75038 and can travel locally, you will have the flexibility to work remotely as you take on some tough challenges. Travel required in local areas about 75% of the time based on business needs. This is a field-based position with a home-based office. Primary Responsibilities: - Assess, plan, and implement care strategies that are individualized by member and directed toward the most appropriate, least restrictive level of care. - Identify and initiate referrals for member transition to community request and provide assistance with social service programs, including financial, psychosocial, community, and state supportive services. - Manage the care plan throughout the continuum of care as a single point of contact. - Communicate with all stakeholders for the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members. - Advocate for members and families as needed to ensure the member’s needs and choices are fully represented and supported by the health care team. - Visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices. Qualifications - Current, unrestricted RN license in the state of Texas. - 2+ years of experience working within the community health setting or in a health care role. - Intermediate level of computer proficiency (including Microsoft Outlook, Teams) and ability to use multiple web applications. - Valid driver’s license, reliable transportation and the ability to travel up to 75% in this ‘assigned region’ to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, nursing facilities or providers' offices. - Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI). - Reside in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service. - Reside in or within a 60 mile commutable driving distance from Dallas County, TX. Preferred Qualifications - 1+ years of experience with long term care services and support, Medicaid or Medicare. - Ability to create, edit, save and send documents, spreadsheets and emails. - Knowledge of the principles of most integrated settings, including federal and State requirements like the federal home and community-based settings regulations. - Reside within one of the following zip codes: 75216, 75237, 75211, 75062, 75061, 75038.
