Role Description These Registered Nurse (RN) positions are located in the VISN 1 Clinical Contact Center and are remote positions. These positions are full-time, 40 hours per week. The Clinical Contact Center (CCC) Registered Nurse (RN) reports to the VISN 1 Clinical Contact Center Nurse Manager. The incumbent provides care and counseling for patients through a virtual care system for the promotion and maintenance of health and prevention of illness. - Provides care through an interactive process between the nurse and patient that occurs over the telephone and online chat technologies and involves identifying the nature and urgency of client health care needs and determining the appropriate disposition. - Provides timely, appropriate advice and guidance to patients and/or their representatives who seek assistance with health care issues. - Communicates through a structured process in which information is exchanged clearly and accurately among team members. - Considers recommended dispositions and interventions from approved clinical decision support tools to assess and address the clinical needs of patients. Recommendations may be emergent and include those that preserve life, prevent disability, or promote health and well-being. - Reports unusual occurrences and adverse events as appropriate. The nurse considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. Role Description This Lead Advanced Medical Support Assistant (AMSA) position is located within the VISN 1 Clinical Contact Center (CCC). Remote work is currently authorized; however, candidates may be subject to return to office in the event policy changes or the exemption is not extended. The incumbent will remain in their current duty station. This position is open to current, permanent employees of the VISN 1 Clinical Contact Center only. The incumbent serves as Lead Advanced Medical Support Assistant (Lead MSA) in a VISN Clinical Contact Center (CCC) as part of an interprofessional healthcare team. Lead MSA provides support across multiple ancillary and interprofessional clinics and determines the needs of the Veteran/caregiver. The lead MSA is responsible for routing clinical questions to appropriate clinical staff. The lead MSA plays an integral role in achieving first-contact resolution through Veteran-centered delivery of care. The Lead MSA provides information on programs and initiatives available to Veterans and provides administrative referrals of services as needed. The Lead MSA collaborates to best coordinate the care needs of the Veteran. - Responsible for daily workload assessments, assigning work assignments; assuring proper staffing coverage. - Evaluate training records; and determine training needs of MSAs to provide support across interprofessional settings. - Assist the unit with complex and non-standard procedures, including clinical flow process related to access to care across multiple clinics, specialties and/or community resources. - Work collaboratively with VISN programs, services, and interprofessional coordinated care team members to provide access to care and meet the needs of Veterans via various available contact modalities to include but not limited to telephone calls, secure messaging, chat, text messaging and video. - Assure coverage of all areas of work, ensuring accurate and timely scheduling of appointments, providing guidance to staff members, to include changes in policies and procedures. - Distribute and balance workload to include: - Create and maintain employee work schedules. - Orient and provide on-the-job training for new and current employees. - Ensure all training requirements are met. - Organize the work structure of assigned areas. - Act as liaison between MSA and staff to resolve day-to-day conflicts. - Scheduling, canceling, re-scheduling patient appointments and/or consults. - Enter no-show information; monitor appointment requests from multiple electronic sources. - Gather and obtain medical information from patients. Qualifications - United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. - English Language Proficiency: Medical Support Assistants appointed to direct patient care positions must be proficient in spoken and written English. - Experience: No experience required. - Education: High school diploma, General Education Development equivalency certificate, or proficiency certificate from a State or territorial-level Board or Department of Education. - Licensure/Certification/Registration: None. - May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Role Description This APN position is in the Clinical Contact Center with the VA Boston Healthcare Systems and is a remote position. This position is a full-time position at 40 hours per week. The Advanced Practice Nurse practices in Veterans Integrated Services Network (VISN) Clinical Contact Center (CCC) to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management through virtual modalities. - The NP in the VISN CCC collaborates with a team of Registered Nurses (RN) to provide clinical care to patients through a virtual triage system. - The NP strives to provide optimal clinically meaningful first contact resolution to patients and seeks end-user feedback to refine their practice. - The NP will communicate with end-users at other patient sites to improve clinical management and integration into Veterans Health Administration (VHA) processes. - The NP practices to the full extent of their education, training, and certification. - Core functions are outlined in the clinical privileges that correspond with this position. - The NP has prescriptive authority for a panel of patients or program as indicated by their position. - The CCC Nurse Practitioner provides over the telephone/VA Video Connect (VVC) and soon to be deployed chat evaluation and management of patients with symptoms. - The CCC provides service to veterans from VISN 1 New England Healthcare System, as well as state of the art telephone care service to veterans 24-hours, 7-days a week. Tele-medicine/VVC duties include, but are not limited to: - Diagnosis and management - Ordering labs and diagnostic tests - Medication prescription and adjustment - Medication reviews and adherence reviews - Patient education Qualifications - A post-master's certificate or master's or doctoral degree as a Nurse Practitioner. - Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency. - Foreign Education: Graduates of foreign schools of professional nursing programs must validate the nurse practitioner degree equivalency. - All APN (NPs) must possess and maintain an active, current, full, and unrestricted APN license to practice as an NP in a State, Territory or Commonwealth of the United States, or in the District of Columbia. - All APN (NPs) must maintain full and current basic board certification in one of the following: Family Nurse Practitioner (FNP), Adult Nurse Practitioner (ANP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), Acute Care Nurse Practitioner (ACNP), Adult-Gerontology Acute Care Nurse Practitioner (AGACNP) or Gerontology Nurse Practitioner (GNP). Role Description This position is located in the Health Information Management Section (HIMS) of the Business Office at the VA Boston Health Care System (VABHCS). The Medical Records Technician (Coder) is responsible for performing a quality review of patient care documents and assigning codes specific for the type of care provided. Duties may include, but are not limited to: - Identifies the principal diagnosis and principal procedure (when applicable) for every inpatient discharge; also identifies significant complications and/or co-morbidities treated or impacting treatment to correctly determine the proper Diagnostic Related Group (DRG). - Conducts re-reviews of codes abstracted for patient encounters (inpatient and outpatient) identified by the VISN VERA committee to determine if based on the documentation the specific VERA coding requirements were followed; corrects coding as needed to ensure proper patient classification in the VERA program. - Codes inpatient professional fee services for identified inpatient admissions in support of the Medical Care Cost Recovery (MCCR) program. - Establishes the primary and secondary diagnosis and procedure codes for billable outpatient encounters following applicable regulations, instructions, and requirements for allowable reimbursement. - Codes all Operating Room procedures reported in the Surgical Package of the Vista hospital system. - Updates codes for current inpatient and Contract Nursing Home admissions for quarterly census and as directed for billable long stay admissions. - Reviews and codes assigned Fee Service patient encounters (inpatient and outpatient) using the paper or electronic documentation obtained from non-VA facilities. Qualifications - Experience: One year of creditable experience equivalent to the next lower grade level. - Demonstrated Knowledge, Skills, and Abilities: - Ability to analyze the health record to identify all pertinent diagnoses and procedures for coding and to evaluate the adequacy of the documentation. - Ability to accurately perform the full scope of outpatient coding, including ambulatory surgical cases, diagnostic studies and procedures, and inpatient facility coding. - Skill in interpreting and adapting health information guidelines that are not completely applicable to the work. Requirements - Citizenship: Citizen of the United States. - Experience and Education: - One year of creditable experience that indicates knowledge of medical terminology, anatomy, physiology, pathophysiology, medical coding, and the structure and format of health records. - An associate's degree from an accredited college or university with a major field of study in health information technology/management. - Completion of an AHIMA approved coding program or other intense coding training program. - Certification: Must have either Apprentice/Associate Level Certification or Mastery Level Certification through AHIMA or AAPC. - English Language Proficiency: Must be proficient in spoken and written English.
