Role Description ThinkWell seeks a highly organized, entrepreneurial, and impact-oriented Project Manager to support our engagement on the Delaware Rural Health Transformation Program (RHTP)—a landmark federal initiative funded by the Centers for Medicare & Medicaid Services (CMS) supporting up to $1 billion in rural health investment over five years. The RHTP aims to transform health care delivery across rural Sussex and Kent Counties, Delaware, through 15 coordinated initiatives spanning crisis stabilization, behavioral health integration, primary care redesign, workforce development, and community-based prevention. The Project Manager will serve as ThinkWell’s primary managerial lead for this engagement and the main point of contact with the state of Delaware’s 15 RHTP subrecipients. This role sits at the intersection of grant compliance, multi-stakeholder coordination, and monitoring and evaluation, with direct exposure to a diverse ecosystem of rural health system partners. Qualifications - Bachelor's degree required. - Master’s degree in public policy (MPP), public health (MPH), health administration, or a closely related field or equivalent experience preferred. - 3-5 years of progressive responsibility in project management. - Demonstrated ability to manage complex, multi-stakeholder workplans, budgets, and deliverable timelines simultaneously. - Strong organizational, analytical, and problem-solving skills. - Outstanding oral and written communication skills. - Ability to work independently and manage multiple priorities in a fast-paced environment. - Ability to travel up to 20% for Delaware site visits and stakeholder engagements. Requirements - Support program coordination across 15 RHTP initiatives, ensuring alignment of activities, timelines, and deliverables. - Plan and schedule regular virtual and in-person meetings, and support knowledge sharing among subrecipients. - Serve as the day-to-day primary point of contact for subrecipients on day-to-day matters, offering guidance on contract management, compliance, and reporting requirements. - Compile consolidated quarterly and annual program data from subrecipients; draft reports that summarize program achievements, challenges, lessons learned, and recommendations for program improvement and sustainability. - Provide quality assurance (QA) and validation checks on data entered into program tracking systems to ensure accuracy, completeness, and alignment with CMS reporting requirements. - Monitor 15 subgrantees’ performance and budget adherence, including monitoring progress against deliverables and conducting subrecipient site reviews. - Conduct periodic audits to ensure alignment with project goals and approved budgets across all 15 RHTP initiatives. - Assist monitoring and evaluation team to ensure that program outputs—including dashboards, reports, and summaries—are clear, accessible, and health-literate for public-facing use. - Complete other duties as assigned in support of the RHTP and ThinkWell’s domestic health portfolio. This role sits at the intersection of grant compliance, multi-stakeholder coordination, and monitoring and evaluation—with direct exposure to state leadership and a diverse ecosystem of rural health system partners. Qualifications - Bachelor’s degree required. - Master’s degree in public policy (MPP), public health (MPH), health administration, or a closely related field preferred. - 5-7 years of progressively responsible experience in project management. - Experience working with state or federal contracts/grants, with a preference for CMS or other HHS agency programs. - Demonstrated ability to develop and manage complex, multi-stakeholder workplans, budgets, and deliverable timelines simultaneously. - Detail-oriented, entrepreneurial, collaborative team player. - Strong organizational, analytical, problem-solving, and written and verbal communication skills. - Ability to work independently and manage multiple priorities in a fast-paced environment. - Ability to travel up to 20% for Delaware site visits and stakeholder engagements. Requirements - Lead program coordination across 15 RHTP initiatives, ensuring alignment of activities, timelines, and deliverables. - Facilitate regular virtual and in-person meetings, cross-initiative communication, and knowledge sharing among subrecipients. - Serve as the day-to-day point of contact between ThinkWell and the State of Delaware, including DPH program staff. - Oversee the development and submission of consolidated quarterly and annual program reports that summarize program achievements, challenges, lessons learned, and recommendations for program improvement and sustainability. - Coordinate with DPH to prepare quarterly and annual reports for submission to CMS. - Oversee periodic audits to ensure alignment with project goals and approved budgets across all 15 RHTP initiatives. - Directly supervise ThinkWell project staff (approximately 1.5 FTE) supporting the RHTP engagement. - Complete other duties as assigned in support of the RHTP and ThinkWell’s domestic health portfolio. Preferred - Experience with rural health initiatives, clinical workforce capacity building, health IT, food is medicine, community health centers, practice transformation and/or behavioral health. - Familiarity with the Delaware health and human services landscape, including DPH, DHSS, or Delaware-based health systems. - Experience using performance management or strategy execution platforms (e.g., AchieveIt) a plus. - Background in monitoring, evaluation, and learning (MEL) in a public health context. - Residence in Delaware or the Washington, DC metro area preferred.

• Provide advisory support, analysis, and implementation assistance aligned with country-specific requests. • Maintain proactive communication with partner institutions and BACKUP Health. • Produce assignment-specific reports and deliverables per TO. • Each TO will specify deliverables, which may include: Assignment work plan and timeline, Technical deliverables (analyses, tools, datasets, training materials), Mission reports and debriefs, Final technical report summarizing outcomes, recommendations, and lessons learned, Handover documents and presentations.

• Provide advisory support, analysis, and implementation assistance aligned with country-specific requests. • Maintain proactive communication with partner institutions and BACKUP Health. • Produce assignment-specific reports and deliverables per TO. • Each TO will specify deliverables, including assignment work plan and timeline, technical deliverables, mission reports, final technical report summarizing outcomes, recommendations, and lessons learned, handover documents, and presentations.

• Respond to country-specific requests related to health governance, community systems strengthening, oversight, or implementation support • Provide technical assistance related to HIV/AIDS, TB, Malaria, and Resilient and Sustainable Systems for Health (RSSH) • Support civil society and community engagement, including: Strengthening the role of civil society organizations (CSOs) and community networks • Supporting community-led and rights-based approaches • Facilitating inclusive and gender-responsive dialogue among communities, CSOs, and government institutions • Engage with Global Fund structures and processes, including CCMs, grant-making, community engagement, and accountability mechanisms • Prepare assignment-specific reports, including recommendations and lessons learned • Maintain communication with partner institutions and BACKUP Health throughout each assignment

• Respond to country-specific requests assigned by partner institutions. • Provide technical advice and inputs on health programme design and integration of HTM services into health systems. • Contribute to evidence-based planning including needs assessments, gap analyses, and programme optimisation. • Engage in multi-stakeholder processes. • Prepare required assignment deliverables including technical inputs, analyses, presentations, and reports. • Maintain regular communication with partner institutions.
