Client Growth Manager

• Serve as the primary point of contact for assigned partner practices • Build strong, trusted relationships with providers, administrators, and clinical staff • Identify and execute opportunities to expand service offerings (RPM, CCM, PCM, RTM, etc.) within existing accounts • Conduct regular performance reviews, presenting data on enrollment, clinical outcomes, and program impact • Ensure client satisfaction, retention, and long-term partnership growth • Schedule and conduct onsite meetings with the assigned partner practices • Partner with Enrollment Team Lead to drive patient enrollment growth and engagement • Monitor key enrollment metrics and proactively address gaps or performance issues • Train and guide practice staff on best practices for patient identification and enrollment workflows • Continuously optimize processes to improve efficiency, compliance, and patient outcomes • Lead onboarding for new partner practices, ensuring a smooth and efficient implementation process • Coordinate cross-functionally with clinical, operations, and billing teams • Provide training and education on program structure, workflows, and expectations • Establish clear success metrics and timelines for program launch and growth • Ensure early adoption and momentum within the first 90 days • Identify and pursue opportunities to bring in new clients within the current territory • Leverage existing relationships, referrals, and market knowledge to generate leads • Support sales conversations with clinical expertise and program knowledge • Collaborate with leadership on territory growth strategy and targeting
Sleep Support Specialist

- Serve a dedicated panel of obstructive sleep apnea patients - Review CPAP reports and communicate with patients during the initial 90 days - Maintain regular communication with patients through phone or video calls - Provide education and coaching on CPAP devices and identify barriers to compliance - Coordinate and order CPAP supplies - Document patient interactions in the clinic's EMR - Collaborate with other health professionals to ensure comprehensive patient care - Stay informed about advancements in respiratory therapy practice and technologies

Position Overview: We are seeking a detail-oriented and proactive professional to join our Enrollment Team and help support our Clinical Team. This multifaceted role requires strong organizational skills, discretion, and the ability to manage multiple priorities across teams. Key Responsibilities - Patient Outreach & Education: Initiate calls to eligible patients, clearly explaining the benefits of our care programs for managing chronic conditions and improving overall health. Assist the care team with patient communication. - Eligibility Verification: Confirm patient eligibility and verify insurance coverage. - Data Entry & Management: Maintain accurate and up-to-date records of patient interactions and maintain documentation to support the integrity of the patient’s chart. - Patient-Centered Communication: Communicate in a professional, empathetic, and engaging manner to build trust and encourage participation. - Team Collaboration: Work closely with internal teams and healthcare providers to ensure seamless coordination and support. - Performance Goals: Consistently meet or exceed established enrollment targets.
