Title: Clinical Pharmacist Location: West Virginia, United States/ VA/ TN Department: Clinical Job Category: Clinical Requisition Number: CLINI006462 remote Full-Time Remote Job Description: The Clinical Pharmacist helps develop and implement Monogram’s medication therapy management (MTM) program which will ensure appropriate use of medication management resources. The Clinical Pharmacist will perform timely written member medication reconciliations and drug therapy action plans for all Monogram members and communicate them to Monogram clinical staff and treating nephrologists, primary care physicians and other providers. He or she conduct telephonic consultations with medical providers when medication changes appear necessary and speak with members about medication use concerns. The pharmacist will work collaboratively with clinical (e.g. nurse and social worker) care manager operations personnel to integrate medication management into Monogram’s overall care model and to ensure that field clinicians are executing on MTM policy and procedures. Roles and Responsibilities - Perform and write concise member specific comprehensive medication reviews (CMRs); productivity and quality standards apply - Drive meaningful medication interventions that result in medical providers’ and members’ adhering to evidence-based medication guidelines - Review member care reports, clinical notes and other medical records as needed for medication action plan development, targeted clinical outreach programs and quality assurance purposes - Serve as a pharmacist resource for Monogram field clinicians regarding medication use and Monogram MTM resources - Assist with the development and delivery of MTM training sessions aimed at building a standardized and effective MTM program for Monogram employees - Assist with the development and maintenance of MTM materials for internal and external provider and member use - Monitor important medication availability and use trends for populations under Monogram’s care management. Stay current with evidence-based prescribing for individuals with kidney disease - Participate in external and internal interdisciplinary care team case rounds or meetings and produce MTM related documentation to be shared with appropriate treating providers - Implement medication optimization processes that meet the needs of managed care organizations and their members - Support Monogram as necessary with any tasks required to deliver excellent personalized kidney care and perform all other duties as assigned - Uphold the mission and values of Monogram Health in all respects Position Requirements - Bachelor’s degree in pharmacy or PharmD - Active pharmacist license and reside in the state of position - 3+ years previous experience (or equivalent) actively practicing as a pharmacist, preferably within a managed care, ambulatory care, or clinical practice environment - Desired qualifications include: - PGY1 residency in ambulatory care, managed care, or related clinical field - BPS board certification in pharmacotherapy, geriatrics, ambulatory care, or related clinical field - Board certification in MTM from NBMTM - MTM certificate training from APhA, Powerpak, or similar program - Office‐based if located in Brentwood, TN or Tempe, AZ with some domestic travel. • Monitor daily clinical quality indicators, trends, and exceptions across all care settings • Ensure adherence to evidence-based clinical standards, policies, workflows, and audit expectations • Provide real-time guidance and escalation support for quality, safety, and workflow concerns • Oversee daily compliance with federal, state, and accreditation requirements (e.g., CMS, state boards) • Lead preparation and readiness efforts for audits; serve as a primary point of contact during reviews • Maintain policies, procedures, and documentation aligned with evolving regulatory standards • Track, investigate, and resolve compliance issues, incidents, and clinical remediation plans • Lead continuous quality improvement initiatives using data, root cause analysis, and best practices • Develop, maintain, and monitor quality dashboards and key metrics • Provide leadership, coaching, and guidance to quality, compliance, and clinical support staff • Support education and training related to quality standards, compliance expectations, and regulatory updates • Communicate quality and compliance performance to executive leadership with clear insights and recommendations • Partner closely with Clinical Operations Audit, Legal, and Compliance to drive aligned and efficient oversight.

• Ensures Post-Hospital Discharge Visits (PHDV) are scheduled within 72 hours of the patients discharge date. • Responsible for scrubbing clinician calendars for the following week ensuring quality visits and optimization of field staff calendars. • Outreach patients with missed visits within the last 3 days to have visit rescheduled. • Completes visit reassignment for clinicians who are not available, including being out of office, transfer to a different role, or have left the Company. • Adheres to scheduling criteria when reassigning the patient to another clinician. • Performs various Pod assignments delegated by direct supervisor on an as needed/requested basis and as time allows. • May be required to provide coverage to other Pods, Regions, and the like based upon business need and may be required to change schedule. • Participates in Pod huddles, which includes Market Manager, MAC, and POD leadership. • Ability to work on site and to adhere to attendance standards.

• Utilize licensing and crisis management skills to assess and address urgent needs. • Assess care plan requirements and assist in their implementation. • Utilize proficiency in documentation and technology to streamline member care. • Offer valuable support and guidance to patients and their families in critical situations. • Primary point of contact for patients, Monogram field staff, and care center personnel needing immediate support. • Provide seamless transitions by providing comprehensive handoffs to incoming staff. • Conduct chart auditing during non-peak call volumes. • Emergency/disaster outreach as events occur. • Support service level agreement (SLA) compliance for annual assessments and care plans. • Assist in post-hospital outreach to ensure patient needs are met, with no outstanding needs that could result in readmissions.

• Manage state licensure, DEA, CDS, and collaborative practice agreements for physicians and other practitioner types (e.g., NPs, RNs, LCSWs) • Prepare clear instructions and correspondence to guide practitioners through application completion and supporting documentation • Proactively follow up and create urgency with practitioners, institutions, and state boards via phone and email to meet start dates and resolve outstanding items • Coordinate and submit required primary source verifications to state boards • Prioritize application workflows based on business needs and departmental guidance • Track licensure progress, document lifecycle milestones, and maintain accurate, timely data in credentialing systems and shared drives • Communicate licensure status and expected timeframes to Licensing & Credentialing, Operations, and Clinical Leadership • Notify Credentialing of license issuances, renewals, and CPA submissions to avoid disruptions in credentialing or enrollment • Generate reports, monitor expiring licenses and certifications, and manage renewals • Perform recurring audits, data reconciliation, and reporting; escalate issues as needed • Ensure practitioner data accuracy across HR, CME, CVO, and credentialing databases • Maintain confidentiality and adhere to Monogram Health credentialing policies

• Manage performance against clinical and operational metrics • Lead, motivate, and drive accountability for a team of nurse practitioners, registered nurses, and social workers • Prepare, review and analyze performance and health outcome reports to identify areas requiring attention and improvement and acting quickly to resolve known issues • Plan staffing schedules to optimize efficiency and productivity • Assist in the hiring of clinical staff members • Execute corporate and regional strategy to deliver quality patient care • Perform other duties as assigned • Uphold the mission and values of Monogram Health in all respects

• Design, implement, and continuously refine standardized workflows across Social Work (SW) and Social Work Coordinator (SWC) functions • Ensure workflows are clearly documented, consistently applied, and scalable across regions and pods • Identify operational inefficiencies and implement process improvements to drive performance and consistency • Ensure workflows, tools, and expectations are clearly understood and executed by frontline staff • Translate training strategy into structured, repeatable training processes and materials • Lead direct management of Social Work Coordinators, including performance expectations, coaching, and accountability • Improve visibility into SWC capacity, productivity, and output • Standardize SWC scope of work and ensure alignment with operational priorities • Build SWC capabilities to support evolving functions • Lead implementation of priority initiatives and pilots • Coordinate across Ops, Clinical, Tech, and Reporting teams to ensure successful execution • Monitor pilot performance, identify barriers, and adjust workflows as needed

• Support chronically ill patients through advanced illness management, palliative care management, and hospice coordination. • Triage urgent palliative care needs and escalate appropriately. • Provide clinical support to regional palliative physicians, including documentation and workflow assistance. • Facilitate hospice referrals and coordinate informational visits. • Support scheduling and pre-visit education for complex patients. • Obtain ACP documentation from external offices and hospitals.

• Engage in initial and continuous outreach to high-risk patients • Educate patients about the Monogram Health program • Ensure patients are accessing program benefits • Drive extensively to visit community members and connect them with care providers • Provide warm handoffs to assigned Monogram Health clinicians and customer support
