This is a work-from-home role with community travel to meet members in their homes and to DCS locations. Mindoula Health is seeking Independent Licensed Therapists to provide remote services for our Washington State Substance Use Disorder Program, supporting substance-exposed members. While a current WA license is preferred, our team will assist qualified candidates in obtaining one. We will also consider applicants licensed in Louisiana, or Florida while we work on the Washington credentialing and licensing. Once licensed, you will work with Washington members on PST hours. Applicants may live anywhere in the U.S. Our program focuses on prenatal and postpartum care and substance-exposed living/pregnancies. You’ll join a family-focused, collaborative team with a flexible schedule, work-from-home autonomy, and the opportunity to create groups and apply your preferred evidence-based practices. In this role, you will provide biopsychosocial assessments, individual and group therapy, and coordinate care across various substance use disorder treatment settings. How you’ll contribute: - Efficiently accept patient handoffs from an Intake Care Manager and seamlessly continue care. - Develop rapport with patients to create a safe therapeutic environment and assist them in developing insight into themselves, their patterns, and their relationships. - Encourage patients to express feelings and discuss what is happening in their lives, assisting them in developing insight into themselves and their relationships. - Guide patients in developing skills and/or strategies for managing problems and triggers to increase relapse prevention and reduce risk. - Encourage patients to develop work skills and to participate in social, recreational, or other therapeutic activities that enhance interpersonal skills and develop social relationships through brief therapeutic interventions including but not limited to motivational interviewing, behavioral activation, and problem-solving therapy. - Educate patients on medication, and the importance of medication adherence, and monitor patients' medication and medication-related problems. Reporting all findings to PCP and Psychiatric Consultants. - Responsible for communicating treatment recommendations, ensuring recommendations are being addressed by PCP, and communicating updates reported by PCP and/or patient to a psychiatrist. - Match daily calendar to documentation in Mindoula's electronic medical record - MMR. - Reflect all billable activity in case of notes on MMR entered within 24 hours after billable activity. - Present follow-ups to the psychiatrist with necessary information during case presentation. - Counsel patients about their mental or physical illness, abuse, medication, and available community and social resources. - Assess patients for risk, make quick evaluations for risk, develop safety plans and respond within the parameters outlined within their safety plan, formulating and performing crisis interventions ensuring the safety of patients as necessary. 
 - Monitor, evaluate, and record the patient’s progress using validated assessments each month that the patient is in the program. - Prepare and maintain all required records, reports, paperwork, and patients’ diagnostic records, maintaining the confidentiality of patients at all times. 
 - Use clear and accurate verbal and written communication skills to facilitate communication with patients, patient's family/caregivers, staff, and the treatment team - Produce high-quality documentation that enables the patients to receive the best treatment possible. 
 - Competently relate to patients, their families, and agencies involved with patients through above-average interpersonal skills. Mindoula's teams of Care Managers, supported by technology including a mobile engagement app, proprietary psychometrics, and predictive analytics, are the key to its success! - Efficiently accept patient handoffs from an Intake Care Manager and seamlessly continue care. - Develop rapport with patients to create a safe therapeutic environment and assist them in developing insight into themselves, their patterns, and their relationships. - Encourage patients to express feelings and discuss what is happening in their lives, assisting them in developing insight into themselves and their relationships. - Guide patients in developing skills and/or strategies for managing problems and triggers to increase relapse prevention and reduce risk. - Encourage patients to develop work skills and to participate in social, recreational, or other therapeutic activities that enhance interpersonal skills and develop social relationships through brief therapeutic interventions including but not limited to motivational interviewing, behavioral activation, and problem-solving therapy. - Educate patients on medication, and the importance of medication adherence, and monitor patients' medication and medication-related problems, reporting all findings to PCP and Psychiatric Consultants. - Responsible for communicating treatment recommendations, ensuring recommendations are being addressed by PCP, and communicating updates reported by PCP and/or patient to a psychiatrist. - Match daily calendar to documentation in Mindoula's electronic medical record - MMR. - Reflect all billable activity in case notes on MMR entered within 24 hours after billable activity. - Present follow-ups to the psychiatrist with necessary information during case presentation. - Counsel patients about their mental or physical illness, abuse, medication, and available community and social resources. - Assess patients for risk, make quick evaluations for risk, develop safety plans and respond within the parameters outlined within their safety plan, formulating and performing crisis interventions ensuring the safety of patients as necessary. - Monitor, evaluate, and record the patient's progress using validated assessments each month that the patient is in the program. - Prepare and maintain all required records, reports, paperwork, and patients' diagnostic records, maintaining the confidentiality of patients at all times. - Use clear and accurate verbal and written communication skills to facilitate communication with patients, patient's family/caregivers, staff, and the treatment team. - Produce high-quality documentation that enables the patients to receive the best treatment possible. - Competently relate to patients, their families, and agencies involved with patients through above-average interpersonal skills. Qualifications - Master's Degree Required in psychology, social work, or another behavioral health discipline from an accredited college or university. - Proficient in technology to successfully work remotely, specifically the Google Suite (calendar, docs, sheets, slides, etc.). - Strong documentation and data entry experience or abilities. - Work experience with Electronic Medical Records. 24/7 Crisis Center Case Manager - 100% Remote Full-time: Monday - Thursday 9 pm to 7am EST This role operates on a scheduled shift within a 24/7 coverage model, including nights, weekends, and holidays. Mindoula is a next-generation population health management company supporting individuals with complex behavioral health, medical, and social needs. Using technology to scale the human connection, our multidisciplinary teams provide 24/7 support to underserved populations, helping members stay safe, connected, and engaged in care. As a 24/7 Crisis Center Case Manager, you’ll be a key member of Mindoula’s crisis response team, providing real-time support to members experiencing behavioral health crises. You’ll respond via phone, text, and chat, assess risk, de-escalate crises, develop safety plans, and coordinate appropriate follow-up care—all while working collaboratively within a multidisciplinary team. Compensation: $23.08 to $24.04 per hour ($48,000 to $50,000 a year) based on education and experience What You’ll Do: Crisis Response & Risk Management - Address and manage crises by responding promptly to calls and digital requests using active listening, de-escalation techniques, and crisis management skills to reduce distress and promote safety. - Evaluate risk type and level (e.g. suicidal ideation, self-harm, homicidal ideation, and other safety concerns) in alignment with established risk-stratification guidelines included escalation and contacting emergency services when indicated - Gather essential information (Member identity, contact information, current location, and—when applicable—parent/guardian details) to support a safe and effective response. - Develop collaborative safety plans with the Member, including coping strategies, use of natural supports, linkage to existing behavioral health providers, and clear next steps for maintaining safety. - Ensure safety plans and crisis resources are sent to the Member promptly and ensure timely communication with leadership and documentation as required. Care Coordination & Follow-Up - Coordinate direct transfer and referrals to appropriate levels of care (e.g., internal, outpatient providers, community resources, mobile crisis teams, emergency departments, inpatient facilities, etc.). - Involve Member’s other support network including family members, guardians, and emergency contacts when appropriate. - Complete timely, accurate, and objective documentation of all contacts, risk assessments, safety planning, and follow-up activities in the designated electronic systems. - Participate in debriefs, case consultations and reviews, and quality-improvement activities related to crisis services. - Perform other duties as assigned.
