Role Description The Triage Coordinator plays a vital role in supporting after-hours patient care by managing calls related to durable medical equipment (DME), escalating visit needs, and scheduling field nurse visits. Reporting to the Telephonic Triage Manager, this position coordinates care activities outside regular branch business hours (nights and weekends), working closely with triage clinicians, branch staff, and leadership to ensure timely delivery of healthcare services that improve patient outcomes. - Handle inbound and outbound calls related to DME, visit escalations, and nurse visit scheduling during after-hours and weekends. - Facilitate timely and appropriate healthcare service delivery outside normal business hours, including care coordination and communication regarding medical needs. - Support the Centralized Triage process by collaborating with triage nurses and branch staff to address immediate patient needs and follow-up care. - Serve as a liaison between the Company, patients, and families to provide compassionate and effective communication. - Collaborate with the treatment team to ensure continuity of care and meet patient satisfaction goals. - Assist in implementing program objectives and maintaining department performance standards. - Maintain accurate, non-clinical records and data entry in compliance with regulatory requirements. - Use multiple computer systems such as CareXM, HCHB, and Microsoft Office for documentation and communication. Role Description We are seeking a Patient Care Manager Specialist to join our team, reporting to the Regional Director of Operational Excellence. Role Description We are seeking a Revenue Cycle Payer Performance and Relations Analyst to join our Revenue Cycle team. This position reports to the Revenue Cycle Analytics Manager and serves as a critical liaison between the contracting and revenue cycle departments. The analyst will lead payer performance reporting, support project management, and ensure alignment between payer requirements and revenue cycle operations. Role Description The Director of Clinical Survey will directly report to the Associate Vice President of Clinical Operations and Survey. This position will assist branches in meeting all applicable regulatory and company requirements in preparation for internal and external inspections, surveys, and reviews (including state licensure, federal certifications, and accreditation). The Director of Clinical Survey will develop, when applicable, and direct the implementation of corrective action plans in branches as a result of unfavorable internal or external survey outcomes with standard deficiencies, or if requested from operational, clinical, compliance or legal leadership. - First responder to ACHC Accreditation Surveys and other external surveys as needed. - Monitor agency level adherence to policies and procedures. - Monitor compliance with patient records and other quality assurance activities. - Coordinate with the appropriate area, regional and corporate staff, including Medical Director, Clinical Manager, Executive Director, Administrators and Area Vice Presidents of Operations on a regular basis to achieve survey success. - Drive agency preparedness in meeting all applicable regulatory and company requirements, by ensuring the implementation of the company’s systems, policies, and standards in preparation for internal and external surveys, and reviews, including state licensure, federal certifications, and accreditation. - Provide expertise to branch directors and staff, as well as up-line management, regarding follow-up action for correction of deficiencies and identification of operational issues. - Provide support, when indicated, for resolution of identified deficiencies in conditional surveys. - Perform other duties as assigned.
