Role Description Evolve Treatment Centers is looking for an enthusiastic Program Aide to join our team at our Virtual Outpatient. Our team provides the highest caliber of treatment to adolescents struggling with substance abuse and mental illness. This position is challenging yet rewarding as our staff have the opportunity to make a significant impact in the lives of teens and their families. The Program Aid position provides a great opportunity for someone who is interested in or currently pursuing a degree in the mental health or social services fields. In this role, you will gain exposure and experience working with teens that have significant mental health and substance abuse issues. Our ideal candidate is dedicated, responsible, reliable, and excited about working with teens with co-occurring disorders, and would enjoy working in a virtual setting. We are looking for someone who is flexible and willing to adjust to the ever-changing nature of adolescent treatment. Job Duties and Responsibilities: - Provide direct care and supervision to clients in the milieu - Provide one-to-one support for clients and address areas where they need support with individual problems - Maintain vigilance and awareness of the peer community dynamic while monitoring the behavior, activities and safety of the clients - Conduct and facilitate psychoeducation groups and recreational activities - Crisis Management and intervention when necessary - Responsible for client group note documentation - Establish a constructive therapeutic community in accordance with Treatment Center policies Qualifications - BA degree or higher in Psychology, Social Work, or related field (preferred) - Some prior experience working in a mental health setting (preferred) - Previous crisis management experience (preferred) - Previous experience working with teens (preferred) Requirements - Must be at least 21 years old or older - Good oral and written communication skills - Possess good personal and professional boundaries appropriate for a treatment setting - Able to remain calm in a crisis situation for the safety of our clients - Able to be fully attentive at all times for client’s safety Benefits - This position pays $21/hr. - Hours are Monday 11am - 7pm, Friday 9am - 5pm - This is a part time, virtual position.

Our California-based programs serve families nationwide and emphasize evidence-based treatment, family involvement, and whole-person care in a supportive environment. We are seeking a Part-Time Child & Adolescent Psychiatrist to provide virtual care within our Partial Hospitalization (PHP) and Intensive Outpatient (IOP) programs. This role involves working across four outpatient programs and collaborating closely with a multidisciplinary team to deliver high-quality psychiatric care. Key Responsibilities: - Provide comprehensive psychiatric care to adolescents in PHP and IOP levels of care, including initial in-depth evaluations completed within 24 hours of admission - Provide weekly clinical support for PHP patients and monthly follow-up for IOP patients - Collaborate closely with families and external providers to ensure coordinated, high-quality care, including participation in peer-to-peer reviews for medical necessity as required by insurance - Maintain timely, accurate documentation, with all evaluations and progress notes completed within 24 hours of service - Work as part of a multidisciplinary team alongside therapists, nursing staff, and program leadership to develop, implement, and adjust individualized treatment plans to provide comprehensive care. Job Summary: The Academic Coordinator serves as the dedicated point-person for all things academic for our clients and families during treatment. This role involves providing tutoring and supervision to clients during dedicated school time, coordinating with schools to ensure clients have schoolwork, linking families with academic resources, and supporting families through the IEP process. This position will work at one 6 bed residential program supporting 6 clients in Danville, CA. Essential Functions - Ability to work as part of a multidisciplinary team to ensure that client’s academic needs are met alongside their mental health needs. - Collaborate with clients and families to develop an individualized academic program including learning goals for each student during their time in RTC. - Facilitates positive learning experiences for clients and provides 1:1 support with schoolwork - Comfortable participating and collaborating in IEP meetings for clients, able to provide parents with assistance in advocating for their child in IEP meetings - Collaborate with parents to get Release of Information forms completed for schools and provide school notes if needed - Understanding of common accommodations schools are able to provide students with learning differences, ADHD, emotional disturbance - Collaborates with school counselors/teachers to ensure client’s academic needs are met. - Maintains safety of students through close observation and monitoring through “line of sight” supervision. - Consults with Primary Therapist, Program Directors, Ed Consultants and parents providing recommendations and accommodations - Provides weekly updates to parents on client’s academic progress - Demonstrates an understanding of reading transcripts and academic credits, alternative school options including GED, virtual school options, community colleges, vocational schools - May create and facilitate weekly study skills groups as needed - Other duties as assigned Qualifications: - Bachelors Degree - Experience tutoring middle or high school students preferred - Customer service experience preferred - Ability to stay calm in crisis situations, possess a passion for working with individuals struggling w/ mental health issues - Must have excellent time management skills with strong attention to detail and strong written and verbal communication skills - Highly proficient in navigating Microsoft Suite and internet based school platforms - Must possess a valid California driver’s license and have a reliable means of transportation - Must be able to pass a LiveScan background check and clear federal and state criminal background checks, and child abuse indexes - Demonstrates knowledge of IEP and 504 plans preferred or willingness to learn This is a Part-Time onsite position - Monday - Thursday 1pm - 5pm(16 hours a week total) $26-28/hr
Non-Exempt- Remote Reports to Aftercare Manager Temp: Maternity Leave Coverage – June 1st – November 15th Payrate: $25/hour Description: The Discharge Coordinator is responsible for academic and discharge planning for clients at the Residential level of care. The Discharge Coordinator will assist the Primary Therapists, Program Directors, and families in identifying and securing the best after-care plan for each client. Aftercare plans can include, but are not limited to, long term treatment programs, Partial Hospitalization Programs (PHP), Intensive Outpatient Programs (IOP), or various other outpatient services (i.e., individual therapy, weekly groups, etc.). The Discharge Coordinator will also follow up with alumni families and encourage them to complete outcomes surveys. This role will involve cross collaboration with the Outreach teams to provide follow up calls and support to families after Discharge. Discharge Responsibilities: - Connect with parents within 7 days of admission to start aftercare coordination - Obtain ROI for client’s school and provide letter of enrollment (if requested) - Collaborate with treatment teams to identify appropriate level of care based on clinical recommendations and program availability in client’s home area(if applicable) - Remain in consistent contact with clinical teams and caregivers throughout the course of treatment to provide effective assistance with discharge preparation - Present parents/guardian with referrals and suggestions for aftercare - Request Releases of Information for outside providers when coordinating after-care plan - Send clinical packets to potential aftercare providers as needed once ROIs are received - Maintain accurate ongoing list of all clients’ anticipated discharge dates - Utilize Salesforce to maintain working database of potential and utilized referrals for inpatient, long-term, wilderness, transport, ed consultants, special ed advocates and outpatient facilities - Ensure Salesforce is current in accurately tracking all discharged clients and their final discharge plan - Collaborate with outreach team to identify referral relationships and new referral options - Meet weekly with each clinical team to discuss discharge plans for clients - Attend weekly stepdown meetings to ensure smooth internal step downs for RTC clients - Act as point of contact to the step-down facilities and provide all requested and necessary documents - Collaborate with the outreach team in connecting with new referents and strengthening relationships with existing referents - Assist families with providing referrals for special ed advocates - Other duties as assigned by Aftercare Manager, Regional Director, or Vice President of Operations Alumni Responsibilities: - Reach out to families post discharge to complete discharge outcomes surveys - Provide referral linkage to alumni families as needed Qualifications: - Bachelor’s Degree preferred and/or relevant experience with case management - Customer service experience preferred - Previous case management experience preferred and/or experience working in Mental Health field - Experience working with IEPs and school districts preferred For information on Evolve's privacy practices, see the Evolve California Personnel Privacy Notice located at https://evolvetreatment.com/personnel-privacy-notice/.
