CHAP

• The Senior Accreditation Manager (SAM) is responsible for assisting the Directors of Accreditation (DA) with daily operations related to accreditation activities. • The SAM works under the direction of the Vice President of Accreditation in conjunction with the Directors of Accreditation to provide exceptional, timely customer service, site visitor education, support, and the continuous improvement of CHAP’s service delivery.

• The Director of Accreditation plays a critical leadership role in ensuring the consistent application of CHAP standards and regulatory requirements across home and community-based care settings. • This position oversees accreditation activities within an assigned specialty and geographic region, collaborates with surveyors (site visitors) and accreditation leaders, and supports decision-making throughout the survey process. • The ideal candidate is a strategic thinker and subject matter expert with deep experience in Medicare-certified home health, hospice, or related community-based healthcare settings. • This individual thrives in a collaborative environment and is passionate about driving quality, compliance, and operational excellence.

• Conducting, planning, organizing, and coordinating healthcare accreditation and/or certification surveys in the community setting • Perform visits in accordance with the CHAP Standards of Excellence • Maintain strict confidentiality unless required by law • Deliver accurate, complete, and well-written documentation
