Role Description The Intake Coordinator is a Registered Professional Nurse who takes referrals and processes appropriate home care services. Responsibilities include: - Maintaining open lines of communication with referral sources and within Americare departments. - Promoting Americare by means of a positive customer service approach. The Patient Relation’s Manager acts as a liaison between the patient, facility and Americare.
