Role Description The Certified Health Coach will work closely with a Dietitian and Clinical Assistant in a fast-paced, technology-driven environment to provide patients with high-quality care regarding Hypertension, Hypotension, Diabetes, Pre-Diabetes, and Obesity. This position will be part of a wider-scope endeavor to test the feasibility and potential impacts of a health coach-facilitated and coordinated care plan. The individual will receive extensive training at the beginning of employment to gain the knowledge and skills needed to work with patients in an individualized manner. The Health Coach will review patient records, gather relevant patient data, meet with referred and enrolled patients, and provide basic health education regarding diet and vitals to support the implementation of the recommendations accepted by the primary care clinician. The Health Coach will consult with the Dietitian, Clinical Assistant, and contracted Primary Care Provider regularly, and act as a representative of the care team when following up with patients. Job Duties - Gather pertinent health metrics, including blood pressure, glucose levels, weight trends, and food diary insights, while reviewing routine screening questionnaires with patients. - Utilize patient-centered coaching methodologies to improve healthy lifestyle behaviors, long-term health outcomes, and quality of life. - Host face-to-face and virtual coaching sessions to assist patients in setting and achieving their medical and lifestyle goals with foundational health education on Hypertension, Diabetes, Pre-Diabetes, and Obesity. - Navigate problem-solving with patients as they encounter obstacles to achieving health-related goals, both during visits and virtually between visits. - Uphold ethical standards and transparency in all actions and decisions, fostering trust with patients, colleagues, and the organization, even when doing so requires extra effort. - Routinely review patients’ vitals data and other health metrics to ensure clinical stability, and provide additional services to assist patients in achieving health control. - Ensure guidance aligns with iHealth’s Care Standards, as well as AHA and ADA recommendations. - Conduct patient outreach calls as needed to ensure patients with low-technological literacy have equal opportunity to receive education, health insights, and set health goals. - Follow up with patients to ensure the implementation of provider and iHealth clinician’s recommendations. - Escalate cases requiring individualized Medical Nutrition Interventions to Registered Dietitians, and to the provider as needed. - Assist the Clinical Assistant in patient onboarding, data entry, scheduling, screening of potential patients, offering technical support, and re-engaging less active patients when necessary. - Prioritize patients above all else, ensuring their needs are addressed promptly—take initiative to identify and address problems, collaborating with others, delegating responsibilities when necessary, and following through to achieve results. - Other duties as assigned by the manager. Qualifications - Bachelor’s degree in Health Sciences, Nutrition, or a related discipline OR an AA with at least 2 years of experience providing health and wellness counseling. - If not already certified, achieve (and maintain) a nationally recognized certification within 12 months of employment, i.e.; NCC or NBC-HWC. - Bilingual proficiency in Spanish/Mandarin/Cantonese/Vietnamese is a plus. - Experience in direct patient care obtaining patient histories, checking vitals, conducting assessments, goal-setting, and motivational interviewing. - Organized, tech-savvy, and highly driven to manage time effectively in a fast-paced, dynamic work environment. - Independent and self-motivated with the experience of working with and appreciating a culturally diverse patient and staff population. - Excellent written and verbal communication skills. - Familiarity with Google suite including drive, calendar, docs, slides, and sheets.
