Role Description MTC Care is the nation’s leading telemedicine provider of behavioral health care to skilled nursing, assisted living and independent living facilities seeking an experienced full-time PCM Care Coordinator. The Care Coordinator is to perform remote monitoring and principal care management for patients under the supervision of the medical team.
