Curana Health
• The Care Manager delivers telephonic care management for Curana patients enrolled in a Value-Based Care Program • Conduct comprehensive assessments of patients’ medical, psychosocial, and functional status. • Review electronic health records (EHR) to identify gaps in care for patients residing in a Long-term Care Nursing Facility. • Review and approve initial and ongoing health questionnaires completed by a member of Curana’s care management team. • Serve as a health coach to educate patients and/or caregivers about their disease process. • Develop patient-centered care plans. • Educate patients and their durable medical power of attorney (DPOA) on the benefits of GUIDE, APCM or CCM. • Support quality gap closure through clinical discovery. • Recommend Provider visits for at-risk patients • Coordinate with the Transitional Care Manager to schedule patient visits and inform the TCM nurse if a patient is discharged to acute or SNF. • Ensure orders, referrals, and prior authorizations are facilitated by the virtual care support team. • Escalate abnormal diagnostic test results to Curana providers. • Acts as the Clinical Liaison for the non-clinical support staff • Communicate patient health updates to the Curana providers. • Communicate treatment plans and health updates to the patient’s caregiver in an effective and caring manner.
Job Requirements
- Must hold an active, unrestricted compact RN license
- Ability to obtain additional state licenses, as needed
- 2+ years of experience in nursing is required
- Care settings may include inpatient, outpatient, or skilled nursing facilities.
- Exhibits knowledge of pathophysiology and accepted treatment protocols for common health diagnoses (i.e., diabetes, chronic heart failure, chronic obstructive pulmonary disease).
- Ability to analyze patient records to identify gaps in care and report to the provider.
- Ability to work in a remote environment that is free of distractions.
- Proficient computer skills and ability to adapt to various technology platforms.
- Excellent written communication skills.
- Demonstrated experience in the usage of clinical data to guide decision making.
- Must have the ability to function independently and as a member of the interdisciplinary care team.

Role Description Reporting to the Michigan State Director, the Campaign Manager works with All Voting state & national staff and relevant coalition partners in support of the Michigan state plan. The work is to establish and coordinate a strategic hub to ensure coordinated preparation for and response to the unique and critical threats to election administration that a major election crisis could pose. The Campaign Manager is responsible for organizing in-state groups and leaders around how election decisions (from legislation to policy) could pave the way for an acute election crisis. The posture can be offensive (e.g.: pushing for expanded ballot access) or defensive (responding to actions taken that would take citizens’ voting rights away.) This position involves work for both All Voting and AVL Action. Core Role Responsibilities - Campaign Development & Management - Implements, develops & executes campaign plans to educate and activate coalition partners and other stakeholders in support of election administration reform, expanding & protecting ballot access, removing barriers to the ballot, countering election sabotage, and voter education in Michigan. - Campaign development is done in coordination with the State Director and Compliance Counsel. - Identifies and develops campaigns to further engage and mobilize partners on voting rights. - Drafts and acts as Owner on campaign plans, presenting them to the larger All Voting team, executing strategies and tactics, and tracking the campaign’s impact. - Partners with comms & policy teams to create digital content, press releases, talking points, etc. - May act as a spokesperson for the state and participate in media events. - Supports State Directors in strategic planning and budgeting. - Tracking the outcomes of our advocacy. - Tracking election timelines and dates, and aligning with / updating All Voting calendar. - Monitoring canvas boards - including creating the program, managing its volunteers, and coordinating subgrantees and/or lawyers. - Election protection -- including poll monitoring and helping to manage voter protection hotline. - Coalition Management - Collaborate and work closely with the existing network of civil and voting rights organizations to mobilize their volunteers in support of campaigns. - Leads education and outreach for campaign initiatives and partners with key allies to identify opportunities for advancement of voting rights in the state. - Expands our network of grassroots partners in state, manages relationships within the state’s coalition, and plays a leading role in representing the organization and acting as lead contact for partners. - Build capacities of our partner organizations & state entities (help them strategize; coach their leaders through building plans). - Manage volunteer base of partner organizations toward a shared larger goal. - Write and manage subgrants, including reporting afterwards. - Organize and execute organization and coalition’s rapid responses to emerging threats. - Co-lead working groups within state voting rights coalition. - Tracking resource gaps among coalition partners; supporting where appropriate. - Policy - Evaluate, propose, recommend and/or design policies for how they impact voters’ rights and help devise solutions to barriers voters face. - Direct Advocacy / Lobbying - Represents All Voting is Local at coalition, governmental, and stakeholder meetings. - Draft/compile research advocacy reports. - Threat monitoring and identification. - Monitoring websites to see if they implement our changes. - Track legislation. - Create trainings on impacts of negative legislation. - Develop policy fixes. - Lead legislative strategy; identify key policy makers, coordinate lobbying efforts, attend relevant committee meetings, create and distribute legislative messaging and talking points for coalition.
