Role Description The RN Triage Nurse at the TX - Our Tyler, Longview and Sulphur Springs Hospice Branches plays a critical role in providing timely and accurate clinical assessments to patients and their families, ensuring appropriate care pathways are followed. This position involves: - Evaluating patient symptoms - Prioritizing care needs - Coordinating with interdisciplinary teams to facilitate effective hospice care delivery - Serving as a vital communication link between patients, families, and healthcare providers - Offering guidance and support during emotionally challenging times By utilizing clinical expertise and compassionate communication, the RN Triage Nurse helps to optimize patient comfort and quality of life. Ultimately, this role contributes significantly to the overall mission of delivering exceptional hospice care tailored to individual patient needs. Qualifications - Current and unrestricted Registered Nurse (RN) license in the state of Texas - Minimum of 2 years clinical nursing experience, preferably in hospice, palliative care, or emergency nursing - Strong knowledge of triage principles, symptom assessment, and patient prioritization - Proficiency in electronic health record (EHR) documentation and clinical communication - Excellent verbal and written communication skills with the ability to provide compassionate patient and family support Requirements - Coordinates care across disciplines to ensure the provision of quality care - Maintains patient confidentiality - Ensures timely communication to all appropriate case team members of documentation - Assesses communications, orders and rehabilitation measures from Clinical Managers to ensure individual needs are met on a patient-by-patient basis - Adheres to plan of care on all assigned patients to provide optimal care - Creates and implements changes to the patient's medical record as needed and agreed upon by the Clinical Manager, DCS, and physician - Performs lead functions and communications related to integration and coordination of care (e.g., facility staff, physicians, pharmacy, case team, supervisor, etc.) - Attends staff meetings and in-services - Performs nursing duties as scheduled and assigned - Provides accurate, timely, complete, and acceptable documentation Benefits - 401K with company match options - Medical Benefits - Life Insurance options - Holiday Pay - PTO

• Operate in a high-volume clinical call center environment managing a combination of inbound and outbound telephonic patient interactions throughout each shift • Conduct outbound phone calls to check in on patients and address health concerns (expected call volume ranges from 70 to 90 calls per day) • Handle inbound phone calls with efficiency and professionalism, triaging and routing appropriately based on clinical urgency in a fast-paced queue-based environment • Route non-clinical inbound calls to the appropriate departments across the company, ensuring minimal hold times and a positive caller experience • Monitor and respond to Remote Patient Monitoring (RPM) alerts in real time, escalating concerns when clinically indicated while managing concurrent call center workflows • Collaborate with providers to coordinate timely and effective patient care via telephonic communication • Perform monthly wellness assessments and complete comprehensive chart reviews • Accurately document all patient interactions in our clinical platform in real time • Consistently meet or exceed individual and team performance metrics related to care quality, patient engagement, call handle times, response times, and adherence to protocol standards • Pivot to different workflows as needed, including RPM Adherence calls, Onboarding calls, and comprehensive chart reviews, particularly toward the end of each month to support departmental goals • Maintain compliance with company policies and applicable regulations • Perform other duties as assigned
