• Partnering with members and families to identify needed supports and direct services to meet personal goals for good health, employment and independent or community living • Collaborates with a team of clinical and social support colleagues to meet the physical, behavioral health and long term service needs of each member • Conduct thorough and objective face-to-face visits with and assess each members situation to determine current status and needs, including physical, behavioral, functional, psycho-social, financial, and employment and independent living expectations • Utilizing criteria for authorizing appropriate home and community based services and confirm those services are being provided and that members needs are being met

• Supports clinical leadership and the care team in patient care activities by providing technical & logistical support within the IKC lane. • Serves as a subject matter expert for resource navigation, and onboarding/training needs. • Schedules & facilitates patient case reviews; Distributes patient list for review and recommendations to the care team after reviews. • Supports quality improvement activities through gathering, tracking, and reporting data as directed by the manager/supervisor and ROM lead. • Acts as subject matter advisor to peers related to navigating eligible health plan programs and resources • Supports onboarding and training activities in conjunction with clinical support CSM

• Secures scheduled encounters by verifying insurance, identifying, and completing payer authorization requirements, creating, and communicating estimated patient liabilities preservice. • Gathers incoming work orders, clinical information, interviews client/referral source to secure demographic, financial, and qualifying data. • Verifies insurance details, documents insurance benefits, produces price estimates, and communicates with patients. • Works collaboratively with insurance companies, providers and staff to ensure plan requirements have been met prior to patient services. • Effectively utilizes medical terminology to collaborate with physicians, case managers, and branch leaders to identify and coordinate patient care needs. • Educates patients and clinical caregivers regarding referral, authorization requirements, payer coverage, eligibility guidelines, and other insurance related changes.
