• Complete all Onboarding requirements within 2 weeks of start date • Meet with your assigned Charlie Health Group Quality Supervisor or Group Quality Director at a minimum of 1x/month for required check-in • Respond to all email and Slack communication promptly (within 48 hours of receiving a communication) • Review the curriculum aligned to group assignment prior to group start time • Arrive ~10 minutes early to scheduled group time and facilitate all groups for the entirety of the hour • Facilitate groups following Charlie Health best practices and using assigned and most up to date Charlie Health curriculum • Facilitate groups across age groups and cohorts, including Integrative curriculum, Support Staffing and Wellness Hour as needed • Foster client engagement and group cohesion throughout the session, encouraging client participation and fostering camera-on culture • Following all operational policies and procedures as indicated by Charlie Health best practices • Participate in collaborative Treatment Team (Tuesdays) and Group Supervision (every other Friday) with your assigned Charlie Health Group Quality Supervisor or Group Quality Director, peers and other Clinical Leadership team members • Work collaboratively and respectfully across the care team including with Primary Therapists, Care Experience Specialists and Care Coaches • Communicate clearly, professionally, and promptly with all clients, staff, families, agencies, and referents • Monitor your treatment documentation to ensure all of your notes are completed within required 24 hour timeframe and meet agency and professional standards per DHCS and The Joint Commission standards • Maintain and model professional ethics, including appropriate boundaries and confidentiality • Other duties as assigned

• Attend all required facilitator trainings and program sessions • Review educational material before each session and prepare as needed on your own time • Facilitate TCP curriculum for program participants in large and/or small group formats depending on assigned role • Coach and mentor participants through large and/or small group facilitation • Provide feedback to staff and LDC volunteers on educational content

• Proactively sources and pre-screens referrals for patient consultations • Assist in the identification of patients who meet the criteria to participate in the IMPACT PLUS program • Initiate patient contact within 2 days of receiving a referral • Gather and organize health information to share across transitions in care • Performs client intake and provide a comprehensive clinical assessment of the patient based on pre-established criteria • Validate, analyze, and interpret information gathered to provide a recommendation of clinical resources required • Ensures the patient, their primary caregiver, and/or family physician are scheduled for the clinic/session within 30 days of the initial referral • Coordinates case conference and participates in the development of the care plan in collaboration with primary care practitioners • Participates in the orientation, training, and education process in this model of care • Supports program development and evaluation, and the sharing of research and knowledge • Ensure all documentation is completed as required for physician records, financial purposes, or as required by individual Health Links

• Lead and support technical teams, ensuring clarity of objectives and effective collaboration • Monitor team performance and delivery health, implementing corrective actions when necessary • Drive continuous improvement initiatives and Agile maturity across teams • Promote the adoption of Artificial Intelligence tools and practices within development teams • Facilitate Agile ceremonies, including Sprint Planning, Daily Stand-ups, Sprint Reviews, and Retrospectives • Facilitate scaled Agile events such as PI Planning, System Demos, and Inspect & Adapt workshops • Coordinate dependencies across teams and support successful release delivery execution • Proactively remove impediments, mitigate risks, and escalate blockers when appropriate • Foster team self-organization, accountability, and continuous learning • Track, manage, and communicate Agile delivery metrics and flow indicators • Serve as a bridge between Product Management, Engineering teams, and organizational leadership • Promote collaboration, transparency, and alignment to ensure successful delivery outcomes
