Treatment will be provided in a remote-based, telehealth format. The behavioral health clinician will provide short-term psychotherapy across the continuum of care, including identifying eligible patients based on defined criteria, providing psychosocial assessment, developing and executing treatment in alliance with patient's goals, and ultimately ending care with a firm discharge plan in place.
